Job summary

Due to the ever-increasing workload, Stroud Cotswold Primary Care Network (PCN) is looking to expand its Social Prescribing Link Worker (SPLW) team of 3, with a particular emphasis on Young People (11- 25 years old) although you may be required to support the team with patients over 25 years old. Social Prescribing empowers people to take control of their health and well-being through referral to non-medical SPLWs who will give time to focus on "what matters to me".

Main duties of the job

The role is primarily to work with Children and Young People (11 - 25 years old) and may involve work with their parents/carers. Social Prescribing empowers people to take control of their health and wellbeing through referral to a non-medical link worker who gives them time to focus on what matters to them.

You will work closely with young people and their families and deliver the right amount of support at the right time.

Job responsibilities

Main duties of the role

Taking and actioning referrals from GPs, Nurses and other practice staff as required.

As part of the PCN Multi-disciplinary team, build relationships with staff in the PCN Practices, attending relevant MDT meetings.

Be proactive in developing strong links with all local agencies to encourage referrals.

Support the PCN requirement to deliver Enhanced Access, delivering social prescribing during the out of hours sessions if the need arises.

Providing personalised support to individuals, their families and carers to take control of their health and wellbeing and to improve their own health outcomes.

Build positive and trusting relationships with young people by giving them time to focus on what matters to them and enabling them to engage in appropriate services and interventions.

Connecting people to appropriate local community groups and statutory services as necessary.

Managing and prioritising your own cases and amongst your colleagues. It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer back to other health professions/agencies when their needs are beyond the scope of the SPLW i.e. when there is a mental health need requiring a qualified practitioner.

Informing and guiding non-clinical and clinical staff within the Network on what services are available within the community and how and when patients can access them

Qualifications

Essential

  • NVQ Level 3 or equivalent.
  • Experience of supporting people, their families and carers including unpaid work.
  • Experience and interest of working with patients aged between 11 and 25 years old.

Experience

  • Interest and experience of working with patients aged between 11 and 25 years old.
  • Valid UK Driving Licence is essential for the role as you will be working across the GP Practices.
  • Experience in the following:
  • Supporting people with mental health either in a paid, unpaid or informal capacity
  • Working with VCSE sector including with volunteers and small community groups.
  • Partnership/collaborative working and of building relationships across a variety of organisations.
  • Working as proactive member of a multi-disciplinary team.
  • Experience of supporting people, their families and carers in a related role including unpaid work.

Desirable

  • Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement including unpaid work.
  • Experience of working as Social Prescriber
  • Training in motivational coaching and interviewing or equivalent experience
  • Experience of working in Primary Care
