Job summary

We are seeking a highly skilled and motivated Emergency Care Practitioner (ECP) or Advanced Clinical Practitioner (ACP)to join our friendly and committed Primary Care team at Lister Medical Centre.

This is an exciting opportunity to work autonomously within a supportive, multidisciplinary environment, delivering high-quality, patient-centred care.

The successful candidate will be responsible for conducting advanced clinical assessments, diagnosing, treating, and managing patients with undifferentiated and undiagnosed conditions.

A strong background in Primary Care is essential, along with excellent interpersonal skills and the ability to work effectively under pressure. Familiarity with SystmOne is desirable.

The post holder will operate within their professional scope of practice and competencies, in line with national and local clinical governance frameworks. They will be expected to:

  • Maintain up-to-date knowledge and skills through continuous professional development.

Main duties of the job

  • Work collaboratively with members of the wider healthcare team, including the District Nursing Team, Mental Health Services, Care Home staff, Ambulance Services, and Secondary Care providers.
  • Adhere to Data Protection legislation and maintain accurate and confidential patient records in line with relevant data collection standards.
  • Promote and safeguard the wellbeing of patients and staff, ensuring all actions align with Practice policies and procedures.
  • Deliver care that meets high standards of clinical governance and is based on up-to-date, evidence-based practice.
  • Manage time effectively and prioritise workload to meet the demands of the service.
  • Identify and escalate any risks to service delivery to the Clinical Lead in a timely manner.
  • Initiate and interpret diagnostic tests and investigations as clinically appropriate.
  • Undertake home visits and care home when required, ensuring continuity and quality of care.
  • Respond to patient complaints in a calm, professional, and courteous manner, aiming to resolve concerns efficiently and satisfactorily at a local level.

About us

Lister Medical Centre is based in Harlow, Essex.

We are a well-established, friendly teaching practice, and our dedicated team includes 8 GPs, 4 Emergency Care Practitioners, 3 Practice Nurses, 2 Nursing Associates, 2 Physician Associate, 4 HCAs and 17 admin/support staff. We have been providing a full range of GP services for over 30 years and have a patient list size of over 21,000 patients.

To practice as an Emergency Care Practitioner (ECP)/ACP and support the on-call team in delivering a same-day emergency care service for patients. The role involves the assessment, diagnosis, treatment, or referral of patients presenting with undifferentiated and undiagnosed conditions. The post holder will operate within their professional scope and competencies.

Key Responsibilities

Clinical

To provide assessment, diagnosis and treatment at first point of contact by attending to patients according to patients needs either in the Practice or patients homes.

To support and develop the Practices same-day emergency care service.

To evaluate clinical information from examination and history taking and initiate appropriate treatment and/or referral.

To signpost appropriately to other services within the defined healthcare pathways.

To refer patients to appropriate care settings, within locally agreed healthcare pathways.

To maintain accurate, timely and systematic documentation of patient interactions and clinical care provided, including adverse events.

To offer patients health promotion advice, support for healthy lifestyles, and disease prevention strategies.

To provide clinical advice and support to other team members where appropriate.

To provide effective chronic disease management for patients, including conducting home visits where required.

Communication

To liaise and interact with multi-professional teams across organisational boundaries, including primary care, social services and secondary care providers according to patient needs and appropriate care pathways.

To maintain communication with all healthcare professionals and outside agencies to ensure seamless, patient-centred service provision.

To ensure effective communication so that the patient receives the appropriate level of care and wherever possible avoid unnecessary hospital admission.
