Clinical Pharmacist

Job summary

Henley SonNet Primary Care network is looking for one or more pharmacists. We have a variety of roles on offer, mostly remote and are flexible about the number of hours and working hours, so please contact us with what you can offer. Ideally we would like:

• Out of hours work eg evenings and/or weekend work, possibly Saturday mornings but could be flexible.
• Some core hours work for individuial surgeries

Main duties of the job

• Carry out medication reviews by phone including structured medication reviews
• Help us achieve targets (eg QOF, IIF, Prescribing Incentive Scheme) which have a prescibing element
• Help with prescription queries.
• Advise on our medication management policies
• Help practices respond to local and national drug guidelines and updates
• Check/action hospital discharge summaries with a prescribing element and update patient records/action accordingly

About us

Henley SonNet PCN is a group of practices in and around Henley-on-Thames. The four practices (Hart, Bell, Nettlebed and Sonning Common) work cohesively and synergistically together to provide joined up care care to our patients.

Generally speaking we prefer our clinical pharmacists to be assigned to individual practices for consistent patient and working relationships although there is collusion among our pharmacists on shared targets and best practice.
