Are you a dedicated and compassionate Practice Nurse seeking a fulfilling opportunity to make a positive impact on the health and wellbeing of your community? We have an exciting opportunity for you to join our team at Cliff Villages Medical Practice, based across Navenby and Waddington.
We are ideally looking for a full-time Practice Nurse (37.5 hours per week, Monday to Friday between 08:0018:30). However, we are open to considering part-time applicants, and compressed hours will also be considered. The post is available as soon as possible.
You will be joining an established practice serving 8,686 patients. Our close-knit clinical team is supported by a very experienced Practice Nurse, two Nurse Practitioners, a Paramedic, two HCAs and a Trainee Nursing Associate, alongside our highly skilled and approachable managerial, reception and administrative teams.
As a Practice Nurse, you will work as part of a multidisciplinary team to deliver high-quality patient care. Ideally, you will be a confident and autonomous practitioner with previous experience in General Practice Nursing, including minor illness and chronic disease management.

Main duties of the job
Provide safe, evidence-based nursing care to patients of all ages.
Carry out baby and childhood immunisations in line with NHS vaccination schedules.
Manage patients with chronic illnesses such as diabetes, COPD and asthma.
Undertake long-term condition reviews, including health checks and lifestyle advice.
Contribute to meeting QOF and Enhanced Services targets.
Maintain accurate and timely patient records using clinical IT systems (e.g. EMIS).
Work collaboratively within the practice nursing team, GPs, and other healthcare professionals to provide patient-centred care.

- Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs
- Long Term Condition Management Following disease-specific algorithms
- Following agreed clinical protocols with referral to GPs or Nurse Practitioners as appropriate

- Travel Immunisations

- ECGs 12 Lead and 24 Hours
- Venepuncture

Other tasks will include:
- Maintaining and cleaning equipment used by the clinical staff

- Ordering of Health Promotion Literature
- Ordering of vaccinations to maintain stock levels
