Are you an experienced and highly motivated Advanced Clinical Practitioner seeking an exciting opportunity? Look no further! We are a group of 4 practices in beautiful Lincolnshire, serving 35,000 patients, and we are on the lookout for a talented individual to join our team.

Main duties of the job

Primarily we are looking for an autonomous Practitioner attending to patients who wish to be seen by a health care professional; diagnosing, treating, and prescribing for patients presenting with undifferentiated and undiagnosed illness or injury assessing their needs, based on highly developed nursing skills including physical assessment. Secondarily, in an ideal world, we would prefer someone who specialises in diabetes management; however, this is not a deal breaker.

Job responsibilities

As an Advanced Nurse Practitioner, you will:

Conduct comprehensive patient assessments, including medical histories and physical examinations

Order and interpret diagnostic tests, including laboratory and radiological tests

Diagnose and treat acute and chronic illnesses, including prescribing medications and treatments

Provide education and counselling to patients on disease prevention, health promotion, and management of chronic conditions

Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care for patients

Maintain accurate and up-to-date patient records

Participate in quality improvement initiatives and clinical research projects

Ensure compliance with all applicable laws, regulations, and standards of practice
