DESCRIPTION
JOB TITLE Practice Nurse
MAIN PURPOSE OF THE POST
To assist medical personnel in the care of our
patient population to include treatment, preventative care, screening and
patient education in the practice and in the local community.
RESPONSIBLE TO:-
The Partners for clinical matters.
The Practice Manager for administrative matters
MAJOR
DUTIES AND RESPONSIBILITIES
Providing
assessment, screening treatment services and health education advice.
1)
Organise
and co-ordinate the provision of nursing services for the practice in liaison
with other members of the nursing team and other Primary Health Care
professionals.
2)
Provide
nursing treatments to patients in participation with general practitioners or
independently, to agreed guidelines and patient group directives
3)
Provide
general and specific health screenings to the patients with referral to general
practitioners as necessary both in the practice and for our patients who are
either housebound or living in Residential and Nursing Homes attached to the
practice
Clinical knowledge in the following:
Wound care/removal of sutures and staples
ECGs
Venepuncture
New patient medicals
Immunisation routine, childhood and travel
Women's health cervical cytology, contraception
Travel medicine
Doppler and compression therapy
Pathological specimens and investigatory procedures
Undertake the collection of pathology specimens, bloods, swabs, smears etc for daily collection
Perform any investigatory procedures requested by the doctors
Treatment
Room supplies and equipment.
1)
In
participation with the Office Manager and other members of the nursing team,
ensure the maintenance of equipment and stock levels relating to patient care.
2)
Maintain
good order of the treatment room.
Pathological
specimens and investigatory procedures
1)
Undertake
the collection of pathology specimens, bloods. swabs, smears etc., for daily
collection at mid-day.
2)
Perform
any investigatory procedures requested by the doctors.
Administrative and
professional responsibilities
To
participate in the administration and professional responsibilities of the
practice team.
1)
Ensure
accurate details of all treatments (where applicable) are recorded in patients
notes.
2)
Ensure
accurate data entries are made on the computer record of any patient contact
and admin issues, including consultations, telephone advice and clinical facts,
e.g. blood pressures, peak flows, cervical status, vaccinations &
immunisations.
3)
Ensure
accurate completion and submission of all administrative forms which need
forwarding to the Business Manager, e.g. Overtime sheets, holiday request
slips, study day requests etc
4)
Ensure
collection and maintenance of statistical information required for regular and
ad hoc reports.
5)
Attend
and participate in practice meetings as required.
6)
Maintain
a notice board in waiting areas designated for patient health care/education as
required.
7)
Ensure
that any information relating to personal details is not discussed or divulged
to any unauthorised person and is regarded as strictly confidential at all
times.
ALL
POST HOLDERS WILL BE EXPECTED TO ADHERE TO THE NHS CODE OF CONDUCT.
Research

1)
Co-operate
and participate as required in any research projects within the practice.
Liaison
1)
Maintain
effective liaison with partners, practice manager, staff and other agencies
concerned with patient care and with all other disciplines within the practice,
with appropriate regard to confidentiality.
Professional
development
1)
Maintain
continued education by attendance at courses and study days as deemed useful or
necessary for professional development.
2)
Engage
with the internal annual appraisal process and maintain a comprehensive PDP.
3)
Complete
Mandatory Training on an annual basis

relationships with patients and colleagues.

The role of the Practice Nurse is an evolving one and it is
anticipated that the post holder will be responsible and reasonably flexible in
the interpretation of the above job description.
Rehabilitation
of Offenders Act 1974
This
post is subject to an exception order under the provisions of the
Rehabilitation of Offenders Act 1974. This stipulates that all previous
convictions, including those that are spent must be declared. Previous
convictions will not necessarily preclude an individual from employment within
the practice, but should be declared in writing at the appropriate stage during
the recruitment process.
