Job summary

We are seeking an enthusiastic and motivated individual for an exciting opportunity as a Care Navigator. You will be responsible for undertaking a wide variety of Care Navigator activities and providing support for the GP practice.

Main duties of the job

The role involves being the first point of contact for patients, ensuring excellent care, communication giving a positive experience. Key duties include greeting patients, managing registrations, handling enquiries, booking appointments, and supporting telephone triage. The position requires efficient administration such as scanning, filing, processing referrals, coding, preparing prescriptions, managing recalls, handling correspondence, and maintaining accurate records. Staff must manage tasks, emails and support clinical teams with chaperoning, specimen handling.

You must be flexible to work across the practices opening hours which are:

Monday 8am to 8pm

Tuesday to Friday 8am to 6.30pm

Saturday's 8.30am to 3pm (rota basis)

Job responsibilities

Some of the job responsibilities include:

Represent the practice positively, as often our care navigation team are the first contact patients will make with the practice- ensuring we offer excellent patient care and experience.

Registering of new patients ensuring all registration forms and documents are fully completed and provided by the patient and that all information is entered appropriately onto the computer.

Accurate recording and timely passing on of messages, ensuring audit trail is maintained.

Effectively handling requests for cancellation and rearranging of appointments.

Completion of various forms when required such as temporary resident forms, etc. and the entering of relevant information onto the computer system.

Making sure all patients are offered excellent care and signposting/triaging to the most suitable service to ensure the appropriate appointments are booked.

Answering the telephone as quickly as ever possible and making appointments on the computer system as appropriate.

Making patient appointments from the online ACCRX consultation facility.

Helping out with duties as required in the event of holidays and sickness; ensuring reception cover at all three sites at all times.

Chaperoning doctors as requested.

Dealing with urine/specimen samples when necessary.

Tidying and restocking of the waiting room and consultation rooms.

Attend training sessions as required and keep Up To Date with mandatory training.

Act at all times with regard to the confidentiality and sensitivity of issues with patients, their care and treatment.
