Job summary
To be part of the city-wide social prescribing initiative
across York based in the community and managed by York CVS. This a dynamic and
forward thinking team advocating for patients whilst working in close
partnership with health and social care to respond to the pressures within the
health and care system.
Link Workers provide
one-to-one support to clients to identify what matters to them and offer
information, advice and support to help them to:
-Explore options to address
non-clinical needs
-Improve their long-term
health and well being
-Address health inequalities
This involves working in a
multi-disciplinary environment and developing networks between the voluntary
and community, health and social care, and primary care sectors.
You may be required to work in other community settings such as a GP's surgery, York District Hospital. You may also be required to undertake home visits in the York area.
The hours for this role will be 30 hours per week, with a salary of £29,179.91 (pro rata).

You will collaborate with local partners to ensure community resources are accessible and thriving, even helping to inspire and establish new initiatives tailored to the needs of the residents of the York City PCN.

About us
As a PCN Social
Prescriber, you will be based within primary care and be part of the York CVS
social prescribing team. You will proactively work with individuals, getting to
know the person, developing a positive relationship with them, understanding
the challenges they face and the changes they want to make. You will work alongside patients to create personalised care
support plans facilitating empowerment and independence.
Referrals
to this service can be complex and will require empathy and innovative
thinking, excellent listening skills and the ability to build strong
relationships whilst maintaining professional boundaries.
You will work collaboratively
with local VCSE, health and social care, and primary and secondary care
organisations, to support people with their health and well being needs.
Establish links and build
working relationships with primary and secondary care colleagues and other
organisations to identify, build on, and support a range of services and
community activities to promote health and well being.
Build and maintain knowledge on
local provision to help promote to clients and others.

- Experience of person-centred service delivery in a community or clinical
- setting e.g. advocacy, support work, counselling or social work.
- The ability to work autonomously and to plan, prioritise work under pressure and adapt to new models of working.
- Extensive experience of using the Microsoft Office suite.
