Job summary
We are seeking an enthusiastic and motivated pharmacist
for an exciting opportunity as a Clinical Pharmacist within the Primary Care
Network. Join our team of experienced Prescribing Clinical Pharmacists,
Pharmacy Technicians and Prescription Clerks.
We aim to provide our pharmacists with variety,
offering opportunities for development and progression within primary care in
York City Centre.
You will work as part of a multi-disciplinary team;
we offer Pharmacists the opportunity to specialise in an area of chronic
disease management.
Alongside usual duties, some of your work may also
be project based to ensure you have an interesting and developmental role.
Main duties of the job
Management of medicine discharge from hospital. -
To reconcile medicines following discharge from hospitals, intermediate care
and into care homes
Manage repeat prescribing, review medicines
reaching review dates and flagging those needing a review with the GP.
Telephone and patient facing - Provide a telephone
help line for patients.
Medication review - Undertake clinical medication
reviews with patients and produce recommendations for the GP on prescribing and
monitoring.
Care home medication reviews - Undertake clinical
medication reviews with patients and produce recommendations for the GP on
prescribing and monitoring. Work with care home staff to improve safety of
medicines ordering and administration.
Undertake clinical medication reviews with patients
and produce recommendations for the GP on prescribing and monitoring.
Long term condition clinics
Care quality commission- Work with the practice to
ensure the practice is compliant with CQC standards.
Provide Medicine information to practice staff and patients.

Manage the repeat prescribing re authorisation process by reviewing patient
requests for repeat prescriptions and reviewing medicines reaching review dates
and flagging up those needing a review with the GP.

Work
with the practice to ensure the practice is compliant with CQC standards where
medicines are involved. Undertake risk assessment and management and ensure
compliance with medicines legislation.
Medicine
information to practice staff and patients - Answers all medicine-related
enquiries from GPs, other practice staff and patients with queries about
medicines.
Monitor
practice prescribing against the local health economy RAG list and make
recommendations to GPs for medicines that should be prescribed by hospital
doctors ( red drugs ) or subject to shared care (amber drugs). Assist practices
in setting and maintaining a practice formulary that is hosted on the practices
computer system. Auditing practices compliance against NICE technology
assessment guidance. Provide newsletter or bulletins on important prescribing
messages.

- Qualifications and experience for junior level practice pharmacists Minimum of 2 years post qualification experience in pharmacy (hospital, primary care or community).

- Qualifications and experience for a senior level practice pharmacist
