Carry out medications reviews, Management of medicines at discharge from hospital, Clinical advice and expertise on medicines to support the team, pharmacological management of long term conditions, medicine safety, audit and quality improvement, medicine audits and antibiotic stewardship
Main duties of the job
Medication Reviews Undertake clinical
medication reviews with patients (including patients in care homes) e interventions including .Management of medicines at discharge
from hospital to
reconcile medicines following discharge from hospital, intermediate care and
into care homes, including identifying and rectifying unexplained changes and
working with patients and community pharmacists to ensure patients receive the
medicines they need post discharge.
Clinical advice and expertise on
medicines to support practice staff, multidisciplinary teams, and patients. Signposting to community pharmacy for
management of minor illness, medicines use reviews. Referring to GPs or other healthcare
professionals where appropriate.
Pharmacological management of long-term conditions: hypertension, hypothyroidism, Vitamin
D deficiency, hyperlipidaemia, osteoporosis using NICE and local prescribing guidance.
Medicines Safety Implement changes to medicines that
result from MHRA alerts, product withdrawal and other local and national
guidance.Audit and Quality Improvement Carry out audits as required by
annual Prescribing Incentive Scheme and Dispensing Services Quality
Scheme.Undertake prescribing and medicines
audits as directed by GPs, feedback the results and implement changes in
conjunction with the practice team.
Antibiotic Stewardship Promotion of evidence-based practice
and interventions to prevent the development of infections to practice.

Marcham Road
Health Centre, Abingdon upon Thames serves a growing population of 14,500
patients. We have a 30-year training history and currently look after GP
trainees and medical students. Our multidisciplinary team consists of GPs, ANP,
Nurses, Health Care Assistants, Social Prescribers, Paramedic, Mental Health
Practitioner, Dispenser, and a full complement of managerial and administrative
staff.
