Job summary

Lytham St Annes PCN are looking to recruit an Experienced General Practice Nurse within the ARRS roles to join our care home team, to help us to transform and modernise pathways of care, enabling the safe and effective sharing of skills across traditional professional boundaries. The post holder will help reduce GP service demand by conducting Care home visits. There are approximately 35 care homes consisting of nursing and residential beds. The care home population is approximately 900. Residents often have multiple comorbidities/Chronic diseases that require regular monitoring and reviews.

The post holder will work to integrate practices and community services to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving access to treatment. The post holder will need to work flexibly to meet the needs of the surgeries within the primary care network.

A Primary Care Network (PCN) enablesthe provision of proactive, accessible, coordinated and integrated primary and community care, improving outcomes for patients. They are formed around natural communities based on GP registered lists, serving populations of around 30,000 to 50,000.

The Lytham St Annes Primary Care Network includes, 5 GP Practices, Holland House, Fernbank, Parcliffe, Poplar and Ansdell, serving a population of 53,772 registered patients.

Main duties of the job

Experienced Practice Nurse with competency in long-term condition management.

Role involves managing long-term conditions and caring for Patients within our Care & Nursing Homes.

Must have own transport and ability to drive.

GP practice experience is desirable, but not essential, including supporting and managing a wider practice team (HCAs, Nursing Associates, Nurses).

Capable of managing Long Term Conditions

Long-Term Condition Management & Elderly Care

Lead and manage long-term conditions such as hypertension, diabetes, asthma, COPD, and cholesterol management.

Provide care for those patients living in Care homes.

Support quality improvement and assurance initiatives within the PCN.

General Practice Nursing Duties within a Care Home Setting

Conduct chronic disease management within the scope of expertise.

Provide immunisations, including:

o Routine influenza, pneumococcal, shingles vaccinations and COVID boosters

Carry out ECGs, venepuncture, urinalysis, and wound care (including ulcer management and suture removal).

Review and action pathology tests such as urine culture and swabs where appropriate.

Chaperone and assist patients being examined by other clinicians.

Leadership & Team Support

Mentor and supervise new-to-practice nurses, nursing associates, and HCAs.

Work collaboratively with the wider practice team to enhance patient care.

Promote public health and screening programs

Integrate population health management approaches to reduce health inequalities.

Clinical Governance & Compliance

Maintain accurate and up-to-date patient records.

Ensure compliance with clinical protocols, including prescribing audits and reviews if qualified.

Adhere to NMC revalidation requirements and practice within the NMC Code of Conduct.

Follow infection control policies and ensure safe handling of specimens.

Contribute to audits and service evaluations.

Support the effective use of NHS resources and practice policies.

Practice & Housekeeping Duties

Ensure the cleanliness and tidiness of nursing and treatment rooms.
