Job summary

Woodlands Medical Practice are looking for an enthusiastic , patient focused and independently motivated registered Nurse Associate or experienced Health Care Assistant to join our friendly and talented team.

You will be an experienced practitioner , who is comfortable working independently and under leadership of our registered Nurses, supporting all aspects of nursing care.

We are seeking an individual who is passionate about delivering high quality care to a wide range of patients.

You should be patient focused independently motivated and posses excellent communication and interpersonal skills.

Main duties of the job

  • Administering injections including childhood Immunisations
  • Support with chronic disease management reviews to include Asthma , COPD, and diabetes.
  • Be competent in venepuncture , ECGs and other diagnostic tests.
  • Assist and perform routine nursing tasks related to patient care
  • Support patients with Learning Disability and severe mental illness
  • Undertake adult and child NHS immunisation programmes
  • Participate in developments within the practice/network and support QOF requirements.
  • Support and assist GPs in minor ops.

About us

Woodlands Medical Practice is a 5 partner GP practice situated in Sutton in Ashfield Nottinghamshire ,with a practice population of around 11,500 patients .

Alongside the the GP partners we have 2 salaried GPs a nursing team comprising of 2 practice nurses 2 HCAS and a phlebotomist.

We are a GP training practice and currently host 5 GP registrars and 1 foundation Doctor.

To compliment the clinical team we have a full Administration and reception team supported by the management team over seeing the day to day running of the practice.

We are part of the Ashfield North PCN and have a share of ARRS staff working within the practice including 3 clinical pharmacists, a Mental Health OT and a muskoskeletal practitioner.

With the support of all the team Woodlands Medical Practice are able to deliver exceptional patient care to our patients and in return as earnt us a CQC rating of outstanding.

Job responsibilities

  • To assist and perform routine nursing tasks related to patient care, such as immunisations, including childhood imms, blood pressure checks, long term condition reviews , including Asthma, COPD, Diabetes, routine diabetic foot checks , inhaler technique checks , immunisations, BMI lifestyle and advice, as directed and agreed with the registered nurses and GPs.
  • To undertake the collection of pathological and microbiology specimens including venepuncture , urine culture in line with local guide lines.
  • To maintain effective control measures in line with local guidelines and practice policy.
  • To support chronic disease management
  • Chaperone and assist patients
  • undertake adult and child NHS immunisations programmes
  • To support health action planning as part of learning disability and severe mental illness physical health check, with the support of practice clinical leads and community specialists.
  • To provide general health promotion education , advice on national screening programmes and guide and refer patients to appropriate services.
  • Ensure accurate notes of all consultations and treatments are reordered in the patients notes and on clinical System SystmONE .
  • Actively take part in mandatory and internal / external training as requested.
  • Any other duty appropriately assigned to you from the Registered Nursing team.
