We have an excellent opportunity for a salaried GP to join our friendly, multidisciplinary team. We are looking for a committed and enthusiastic GP to support the delivery of highquality healthcare and positive outcomes for our patients.
The role is flexible, with 6 to 8 sessions per week available, and we are happy to discuss sessional arrangements to suit the right candidate.
We are seeking a selfmotivated GP who is confident working independently, while also valuing and contributing to the wider team. The ideal candidate will be forwardthinking, open to change, and committed to balancing innovation with the needs of both patients and colleagues.

Main duties of the job
You will deliver high quality clinical care to patients through a range of consultation methods, including face to face, telephone, and remote/e-consultation appointments. The practice operates a total triage model to ensure patients are directed to the most appropriate care in a timely manner.
The role includes participation in oncall and triage duties on a prorata basis, with an expectation that you will contribute equitably to home visiting. In addition, the practice provides extended access services, and you will be expected to cover one extended access session per week.
You will be responsible for the provision of comprehensive medical services to our registered patient population, managing a personal caseload across a broad spectrum of acute and chronic health needs. You will work collaboratively within a supportive, multidisciplinary team and will both support and be supported by a wide range of experienced clinical and nonclinical colleagues within the practice.

We have a team of over 90 staff members, including GP partners, salaried GPs, and a wide multidisciplinary team comprising Advanced Clinical Practitioners, First Contact Practitioners, Mental Health Practitioners, clinical pharmacists, nurses, and other allied healthcare professionals. This is supported by a strong nonclinical team who ensure the smooth day to day running of the practice. Our current patient list size is approximately 27,000.
We are a large, forward thinking training practice operating across three sites located in Chaddesden, Borrowash, and the University of Derby. Our practices are rated Good by the Care Quality Commission and have been awarded the RCGP Quality Practice Award on two occasions. Clinical Responsibilities

In
accordance with the practice rota, as agreed, to undertake a variety of duties
including clinical sessions (number tbc) comprising surgery consultations,
telephone and e-consultations and queries, visiting patients at home, checking and
signing repeat prescriptions and dealing with queries, paperwork, reports and
correspondence in a timely fashion and in accordance with the contractual
obligations set out by the GMS contract.
To
make professional, autonomous decisions in relation to presenting problems,
whether self-referred or referred from other health care workers within the
organisation.
To
assess the health care needs of patients with undifferentiated and undiagnosed
problems.
To
screen patients for disease risk factors and early signs of illness.
In
consultation with patients and in line with current practice disease management
protocols, to develop care plans for health.
To
provide counselling and health education as appropriate.
To
admit or discharge patients to and from the caseload, and refer to other care
providers as appropriate.
To
record clear and contemporaneous consultation notes to agreed standards.
To
participate actively in the collection of data for audit purposes.
To
compile and issue acute and repeat prescriptions.
To
prescribe, in accordance with the practice prescribing formulary, all local and
national guidelines, appropriately for the presenting condition. Should it be
clinically indicated to prescribe outside the guidelines, a full explanation of
the reason must be given in the patients medical record and the appropriate information
given to the practice manager to enable flagging to medicine management.
To
provide support for other clinicians such as nurses, nurse practitioners and
health care assistants including the signing of prescriptions where
appropriate.
To
work with other clinicians to establish a culture of learning and professional
development and to take part in the training of undergraduates, General
Practitioners and other healthcare professionals.
