Job summary
The Triage Nurse role is integral to ensuring that all patient requests into the practice are clinically assessed and prioritised accordingly. The post holder will provide self-care advice appropriate to the patients needs or allocate them to the most appropriate practitioner or service.
Main duties of the job
Working as part of the Triage team, the post holder will provide clinical triage of all e-consultations, telephone calls and home visit requests where needed.
The post holder will identify the clinical need and urgency to provide a time frame for intervention, allocating them to the most appropriate clinician or service to meet the needs and will provide advice and signposting as required.
There may be, on occasion, a requirement to carry out other tasks.
