Job summary

An exciting opportunity has arisen for an experienced, enthusiastic and highly motivated Practice Nurse to be part of our friendly practice team, with a patient list size of 8000 patients. The applicant will be responsible for ensuring that the practice provides an optimum service of nursing care relevant to the patient population; working to agreed protocols.

We are looking for an experienced and enthusiastic Practice Nurse to join our friendly primary care team on a full-time, fixed-term basis (18 months).

The successful candidate will play a key role in delivering high-quality nursing care to our practice population, with a particular focus on chronic disease management and womens health.

Main duties of the job

  • Conduct Chronic Disease management COPD, Hypertension, Asthma and Diabetes, providing evidence-based management and patient education.
  • Perform cervical cytology (smear tests) in line with national screening guidelines.
  • Spirometry Interpretation.
  • Support the management of long-term conditions and contribute to QOF targets.
  • Provide general treatment room duties, including wound care, immunisations, and health promotion.
  • Work collaboratively with GPs, HCAs, and the wider multidisciplinary team to deliver excellent patient care.
  • Initiating and maintaining health screening and promotion clinics for identified areas of the practice population.
  • Deliver child immunisations and vaccinations, adult vaccinations including travel vaccinations.

Essential Requirements

  • Registered Nurse (NMC Registration required)
  • Primary Care experience (minimum 1 year)
  • Competent in COPD and Asthma reviews
  • Trained and competent in Cervical Cytology
  • Excellent communication and organisational skills
  • Wound care management.

Desirable

  • Experience with SystmOne
  • Additional chronic disease management qualifications
