Job responsibilities

• Job Description

• Conduct Chronic Disease management COPD, Hypertension, Asthma and Diabetes, providing evidence-based management and patient education.
• Perform cervical cytology smear testsin line with national screening guidelines.
• Spirometry Interpretation.
• Support the management of long-term conditions and contribute to QOF targets.
• Provide general treatment room duties, including wound care, immunisations, and health promotion.
• Work collaboratively with GPs, HCAs, and the wider multidisciplinary team to deliver excellent patient care.
• Initiating and maintaining health screening and promotion clinics for identified areas of the practice population.
• Deliver child immunisations and vaccinations, adult vaccinations including travel vaccinations.
• Essential Requirements
• Registered Nurse NMC Registration required
• Primary Care experiencea minimum 1 year
• Competent in COPD and Asthma reviews
• Trained and competent in Cervical Cytology
• Excellent communication and organisational skills
• Wound care management.
• Desirable
• Experience with SystmOne
• Additional chronic disease management qualifications

• Commitment to professional development and revalidation.
• Enhanced Disclosure Barring Service DBS check.
• Flexibility to work outside core hours if required.

Qualifications

Essential

• Registered Nurse with current registration with the Nursing & Midwifery Council NMC.
• Evidence of continued professional development CPD and meeting revalidation requirements.

Desirable

• Postregistration qualification or diploma or degree in Practice Nursing, Primary Care or chronic disease management.
• Independent Nurse Prescriber qualification.

Experience

Essential

• Experience working in primary care or general practice or similar community nursing environment.
• Experience managing long-term conditions e.g. diabetes, asthma, COPD, CHD.
• Experience of working autonomously and as part of a team; using clinical systems; delivering routine nursing treatments.
• Experience of cervical screening/cytology; immunisations childhood, travel, quality initiatives and audit, infection prevention & control

Desirable

• Experience of using specific systems, SystmOne
• Ability to use IT systems and maintain accurate records.
