Job summary

We are seeking a skilled and dedicated Practice Nurse with a specialism in diabetes care to join our team. The successful candidate will play a crucial role in the delivery of comprehensive diabetic care to our cohort of 900 diabetic patients. This role involves conducting diabetic reviews, supporting patients with diabetes management, and working collaboratively with other healthcare professionals, including GPs and Healthcare Assistants (HCAs), to ensure holistic care.

Main duties of the job

- Conduct routine and annual diabetic reviews in line with national guidelines (e.g., NICE) and local protocols.

- Develop and implement individualised care plans for patients with diabetes, including medication reviews, dietary advice, and lifestyle management.

- Monitor and manage diabetes-related complications such as foot care, high cholesterol and blood pressure, referring patients to specialists as needed.

- Educate and support patients in blood glucose monitoring, insulin administration, and self-care strategies to optimize diabetes control.

- Assist in the organization of diabetes clinics and provide clinical leadership in diabetes care within the practice.

About us

Linden Hall Surgery has two sites, one in the Shropshire market town of Newport, and another in Muxton, Telford, with a combined list size of 15,500 patients.

The surgery consists of a close-knit team of 4 GP partners and 5 salaried GPs, alongside 4 Practice Nurses and 3 HCAs. We also have an expanding team of associate healthcare professionals including a First Contact Physiotherapist, a Clinical Pharmacist, Paramedics, Pharmacy Technicians, Social Prescribers, a Mental Health Practitioner, and an Age UK Link Worker. The clinical team is very ably supported by an extensive & experienced administrative team.

We are a total triage practice, with all patient contact coming through our online triage platform. All requests are reviewed by a GP who allocates the patient an appointment with an appropriate team member within a suitable timescale. More than 75% of appointments are delivered within 48 hours of contact, and all within 2 weeks.

The surgery has a strong track record of chronic disease management, and despite our growing size we pride ourselves on personally knowing our patients and maintaining traditional family medicine. With the prevalence of diabetes continuing to rise in particular we want to ensure our patients receive the very best care.
