NEW HOME VISITING SERVICE
Brookroyd is seeking a
motivated salaried GP,ANP,Paramedic,pharmacist or any other suitability qualified clinician to join our Practice for up to 5 sessions a week . These 5 sessions would be providing our home visiting service . When there are no visits required the time would be utilised in house providing a mixture of face to face, telephone and online consultations.
Acute Care Team of
GPs, GP registrar, Advanced Nurse Practitioners ,Practice Nurses, Healthcare Assistants, Phlebomists
Pharmacist , reception staff trained for medication and prescribing
Part of SHAWN PCN, variety of additional ARRS roles embedded into the practice team
Active signposting by
reception to ensure patients are seen by the right person at the right time
Established training
practice including GP trainees and Leeds medical students

Main duties of the job
The role will require visiting our patients in their home environment. Initial
history taking, clinical assessment, diagnosis, treatment and evaluation of
care. Be able to demonstrate safe, clinical decision-making and expert care,
including assessment and diagnostic skills, The post holder will demonstrate
critical thinking in clinical decision-making and work collaboratively with the
general practice team to meet the needs of the patients. Delivery of policy and
procedures, and providing appropriate leadership in accordance with the GMC. We
are looking for a hard-working, committed clinician who will play an active role
within our team to maintain our values and help develop patient services in the
future.

Who
we are: Situated in Heckmondwike,
a market town, Brookroyd Limited is on the commuter belt, access to local
amenities and close to excellent schools, including Heckmondwike
Grammar school and is easily accessible from Leeds, Bradford, Calderdale
and Huddersfield. We are an established training practice with a list size of
approximately 10,000 patients and are rated Good by CQC.
Alongside our experienced Doctor team; we also have a PCN Pharmacist, Social prescriber, physiotherapy services, Diabetic Nurse Specialist , Diabetic Dietician ,Care coordinator and a Nursing team
that lead on Chronic Disease management.
Staff well-being is important to our Management team. Therefore we work SMART and keep everyone's workload manageable. We have highly skilled and experienced administration and reception staff supporting our clinical team.

Essential
- FULLY QUALIFIED GP or ANP-Essential Criteria detailed below
- Fully Qualified GP. Full GMC Registration.
- On Performers List or eligible to apply. Medical Degree MRCGP

- Masters Degree in Advanced Clinical Practice or equivalent.
- Current Registered Nurse (NMC)
- Hold a current valid driving license.
- Independent Prescriber.
- Minor Illness Diploma or equivalent level of qualification/experience.
- Proven ability of the safe management of acute and ongoing illness in patients presenting with undifferentiated diagnoses.
- Experience of delivering care either face to face, by telephone, video and in a patients own home. Directing care appropriately and managing complete episodes of care independently.
- Able to work autonomously, without immediate support and supervision.
- Valid UK Work permit (if needed).
- DBS clearance
- Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and on-going competence
Desirable
- Experience of System 1 and Ardens.
- Two years working in a primary care or an urgent care environment.
- Knowledge of clinical audits.
