Job summary
The Care Coordinator is a crucial role within
the GP practice and the purpose of the role is to reduce GP Practice Workload,
help improve access times for patients and facilitate the effective and
efficient referral of patients to external service partners and to reduce clinical
variation across the network by adopting agreed standard clinical protocols.
This role will offer
flexible and proactive support to vulnerable patients in relation to their
health-related appointments and interventions.
Main duties of the job
They will work closely with GPs and
practice teams, making sure that appropriate support is made available to
people; supporting them to understand and manage their condition and ensuring
their changing needs are addressed. They will enable people to access the
services and support they require to meet their health and wellbeing needs,
helping to improve peoples quality of life.
The postholder will work with a diverse
range of people from different cultural and social backgrounds. The ability to
work confidently and effectively in a varied, and sometimes challenging
environment is essential.

skills, and be organised, patient and empathetic. They will have experience of

About us
The Goodinge Group Practice puts patient care at the heart of what it does and maintains the values of teamwork, professionalism and compassion in providing traditional family medicine to the local community. The practice is a GP training practice with two GP trainers. We also provide supervision and training for clinical and non-clinical staff members.
The Partnership are keen to develop the practice and innovate to meet the growing challenges of the NHS. We have a medicines management team and structured administrative team. The practice is also part of the organisation WISH, which provides GPs to the UCC at the Whittington and works closely with the Islington Federation.

- Knowledge of EMIS/System One
- Able to work with Microsoft Office and 365

- NVQ Level 3 in adult care - advanced level or equivalent qualifications or working towards
