Job summary
Staverton Medical Practice in Kilburn is looking for an experienced Advanced Nurse Practitioner to join our friendly and supportive team. You will provide high-quality, patient-centred care to people with a wide range of health needs, from initial assessment and diagnosis through to treatment and follow-up. Working autonomously and collaboratively with our GPs and wider clinical team, youll use your advanced clinical and prescribing skills to manage acute and long-term conditions, promote health and wellbeing, and contribute to service development within the practice.
Main duties of the job
As an Advanced Nurse Practitioner (ANP) at Staverton Medical Practice, youll provide safe, effective and compassionate care for patients of all ages. Working autonomously and within our multidisciplinary team, youll assess, diagnose, treat and manage a wide range of acute and long-term conditions.
Youll act as a first contact clinician, delivering expert clinical assessment, diagnosis, prescribing and evidence-based management of undifferentiated or complex health problems. Youll support patients in managing both minor and chronic conditions, promote self-care and healthy lifestyles, and contribute to our chronic disease management and same-day access services.
Key duties include:

Work with patients to support compliance with and adherence to prescribed treatments Provide information and advice on prescribed or over the counter medication on medication regimens side effects and interactions
Prioritise health problems and intervene appropriately to assist the patient in complex urgent or emergency situations including initiation of effective emergency care
Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self care
Communication
Use and demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment
Communicate with and support patients who are receiving bad news
Communicate effectively with patients and carers recognising the need for alternative methods of communication to overcome different levels of understanding cultural background and preferred ways of communicating
