Job summary
We are looking for a practice nurse to join our friendly team at Chelford Surgery. The role will involve a mix of clinical and managerial skills within the nursing team.
The Practice Nurse will be responsible for a number of clinical areas such as chronic disease management (particularly respiratory disease), infection prevention and control and vaccination and immunisations.
The practice is actively involved in research and there is also the option to support the lead GP in delivering research within primary care.
The practice nurse will be expected to provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken by an experienced RGN and in addition any roles agreed between the nurse and the doctors as appropriate, having regard to current training / regulations.

Management of the practice respiratory disease patient register
Assess, plan, develop, implement, and evaluate programmes to promote health and
well-being, and prevent adverse effects on health and well-being.
Implement and evaluate individual treatment plans for patients with a known long-term condition
Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition.
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
Support patients to adopt health promotion strategies that encourage patients to live.
healthily, and apply principles of self-care.
Provide information and advice on prescribed or over-the-counter medication on
medication regimens, side effects and interactions
Support and advise women requesting information relating to family planning needs.
Support and manage health needs of women presenting for cervical cytology consultations
Provide a lead role in Vaccinations and Immunisations (childhood and adulthood)
Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions.
Provide a holistic approach to wound management and implement wound care in line with current evidence based guidelines.

Able to recognize and manage anaphylaxis according to current UK guidelines.
Able to perform Cardio-pulmonary resuscitation according to current UK guidelines.
Assist in the provision of minor surgery.
Ability to monitor and manage maintenance of stock and equipment.
To engage and take responsibility for professional development and training when required.
Attend mandatory training and updates as well in house yearly appraisal.
Chelford Surgery, based in East Cheshire, has a growing list size of circa 4,500 patients. We are an established rural Training and Dispensing Practice with 3 Partners. We have an extended team which includes 4 salaried GPs, a nursing team of 2 additional Practice Nurses and 2 Healthcare Assistants. We have an onsite dispensary and a full complement of administrative and support staff.
Through our PCN, we have developed a team which includes, clinical pharmacists, first contact physiotherapists, social prescribers and other additional roles.
