Job summary
Due to retirement, we are looking for an additional experienced and
motivated Health Care Assistant to join our enthusiastic and friendly
healthcare team, comprising of two practice nurses and two health care
assistants. Previous experience is essential, however continued professional
development and training will be encouraged/provided for the successful
candidate.
We are a friendly, forward-thinking practice with an
excellent nursing team, stable admin team, high QOF achievement, and are CQC rated
good. We operate EMIS web clinical systems and are paper light utilising Docman.

Main duties of the job
The successful candidate will be required to:
Undertake phlebotomy & dressing clinics.
Undertake specific HCA clinics as required including
BP monitoring, New Patient Medical & NHS Health checks, ECGs, B12 injections
and Influenza/pneumonia vaccinations where clinically indicated.
In addition to your clinical duties you may also be
responsible for the general treatment room duties normally expected of an HCA
eg. stocking of clinical areas and ordering supplies.
You will have a named staff member as your mentor.
You will work closely with the nursing team to
develop the service to meet the future needs of the practice in providing disease
treatment and health promotion services.
You will be expected to maintain your own skills and
knowledge necessary to perform the job to the standard expected of a high
performing team HCA.
You should be available to attend away days for mandatory training and attend study days as necessary to update
and increase knowledge and skills.

Chelford Surgery operates a total triage model of
care and is actively engaged in its local Primary Care Network (PCN) CHAW,
comprising of four other local surgeries. We are supported by additional
clinical roles staff including a Physicians Associate, Musculoskeletal

To assist medical personnel in the care of practice
patients to include treatment, preventative care, screening and patient
education. The post will also include some limited administrative duties.
You will act as a focal point of communication
between patients, doctors, nurses and other members of the primary health care
team.
General
To maintain a safe and effective level of practice
and to bring any deficits in training to the attention of the manager so these
can be addressed.
You will be required to always act in a professional
manner and adhere to practice policies, protocols and guidelines.
Training
Training requirements will be monitored by yearly
appraisal and review. Personal development will be encouraged and supported by
the practice. It is the individuals responsibility to remain up to date with
recent developments.
Accountability
Although accountable to the Partners of the practice
through the Practice Manager as an employee, you are ultimately responsible for
your own practice and must ensure that limitations are always acknowledged.
Full training will be provided for any clinical procedures.
Administration
All records need to be kept clearly and legibly.
Claims are an important part of practice income and are a priority, needing to
be made correctly and accurately. A great deal of this work will be done on the
computer.
Meetings
It will be necessary to attend various Practice
meetings as requested.
Liaison
As well as the nursing team, the HCA will work
closely with reception and dispensary staff and will be expected to communicate
and liaise with all other members of the Practice team.

Ensure accurate and legible notes of all
consultations and treatments are recorded within the patients medical notes on the computer
system.
Ensure the clinical computer system is kept up to
date with accurate details recorded.
Ensure accurate completion of all necessary
documentation associated with patient healthcare and registration with the
practice.
Ensure collection and maintenance of statistical
information required for regular and ad hoc reports as deemed part of the role.
Management of appointment system
Ensure total familiarity with all appointment
systems including regular and incidental variations, such as systems used for
extended hours.
Any other delegated duties considered appropriate to
the post.
