Introduction to the Role:We are seeking a dedicated and knowledgeable Prescribing Pharmacist to join our Primary Care Network (PCN) team. The ideal candidate will play a vital role in ensuring the safe and effective use of medications for patients within the PCN. This role requires a strong understanding of pharmacology, patient care, and the ability to work collaboratively within a multidisciplinary team. The successful candidate will be responsible for providing independent prescribing services, conducting medication reviews, and optimizing pharmacotherapy to improve patient outcomes. Main duties of the job
Duties of the Main Role:
- Deliver independent prescribing services with a focus on patient-centered care.
- Review and evaluate medication orders for appropriateness, safety, and efficacy.
- Provide medication administration guidance to healthcare professionals and patients.
- Conduct patient assessments to monitor medication therapy outcomes.
- Collaborate with physicians and other healthcare providers to optimize pharmacotherapy.
- Educate patients about their medications, including potential side effects and interactions.
- Maintain accurate records of medication administration and patient care activities.
- Stay updated on current medications, treatment protocols, and clinical guidelines.
- Deliver a wide range of projects including reviews for long-term condition management, structured medication reviews, managing patients on high-risk medication, processing medication alerts and supply shortages, medication reconciliation, patient safety projects, and clinical audits to help drive quality and efficiency.
