Introduction to the Role:We are seeking a dedicated and knowledgeable Prescribing Pharmacist to join our Primary Care Network (PCN) team. The ideal candidate will play a vital role in ensuring the safe and effective use of medications for patients within the PCN. This role requires a strong understanding of pharmacology, patient care, and the ability to work collaboratively within a multidisciplinary team. The successful candidate will be responsible for providing independent prescribing services, conducting medication reviews, and optimizing pharmacotherapy to improve patient outcomes. Main duties of the job

  • Deliver independent prescribing services with a focus on patient-centered care.
  • Review and evaluate medication orders for appropriateness, safety, and efficacy.
  • Provide medication administration guidance to healthcare professionals and patients.
  • Conduct patient assessments to monitor medication therapy outcomes.
  • Collaborate with physicians and other healthcare providers to optimize pharmacotherapy.
  • Educate patients about their medications, including potential side effects and interactions.
  • Maintain accurate records of medication administration and patient care activities.
  • Stay updated on current medications, treatment protocols, and clinical guidelines.
  • Deliver a wide range of projects including reviews for long-term condition management, structured medication reviews, managing patients on high-risk medication, processing medication alerts and supply shortages, medication reconciliation, patient safety projects, and clinical audits to help drive quality and efficiency.

  • Registration with the General Pharmaceutical Council (GPhC).
  • Independent Prescribing qualification.
