Job summary

Latham House Medical Practice, one of the UKs largest doctors surgeries, are looking for a committed, innovative, and enthusiastic Cancer Care Coordinator to join our semi-rural Practice based in the town centre of Melton Mowbray.

Main duties of the job

Care Coordinators provide extra time, capacity and expertise to support patients and staff to ensure patients receive timely personalised care. You will work closely with the GPs and other clinicians to identify and manage a caseload of patients, ensure the robust and effective Care Coordinator activity from the diagnosis of cancer and ongoing support thereafter. You will also work cohesively with the other Care Coordinators in the Practice.

Your primary focus will be supporting the multidisciplinary teams and coordinating the pathway for patients with potential cancer and for those who have received a diagnosis. The Cancer Care Coordinator will be working with patients to help ensure they have the right support to understand their diagnosis, what the next steps are and match the support that is available locally to help them during this distressing time.

Job responsibilities

The Cancer Care Coordinator will:

- Send 2 Week Wait referrals, ensuring the results of required preliminary investigations are including in line with the PRISM referral pathways

- Ensure that all patients receive information on their referral including why they are being referred, the importance of attending appointments and the contact information for the hospital they are being referred to

- Be responsible for contacting patients one week after referral for a welfare check and to ensure an appointment has been received as part of the NICE guidelines safety netting process

- Ensure the patient receives their appointment, chasing referral outcomes with the 2 Week Wait office where required

- Run weekly searches on SystmOne to ensure all referrals have been sent and identify any with outstanding actions

- Bring together all of a persons identified care and support needs and explore their options to meet these into a single personalised care and support plan, in line with PCSP best practice

- Raise awareness of shared decision making and decision support tools and assist people to be more prepared to have a shared decision making conversation

- Acting as their primary contact within the Practice, help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care

- Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level.

- Provide coordination and navigation for people and their carers across health and care services, alongside working closing with the PCN social prescribing link workers, health and wellbeing coaches and other primary care roles

- Support the coordination and delivery of MDTs within PCNs

- Liaise with external organisations where necessary

- General administration and note taking as appropriate
