Main duties of the job
The post holder is responsible for the delivery of practice nursing services care to the practice population. Supported by senior nurses within the practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, in accordance with practice protocols and local / national guidelines.

About us
We are a supportive, forward thinking team. The practice has a list size of just over 12,500 patients and a strong teaching ethos.
The clinical team is made up of GP partners, Advanced nurse practitioner, Practice nurses, HCA's and Trainee Nursing Associates. We also have excellent support from the administration team. The whole team is encouraged to contribute to the provision of high standards of care to patients and their families.
