Job summary

We are looking for a Practice Nurse with a special interest in Diabetes

An exciting opportunity has arisen to join our friendly team serving 21,300 patients from our GP practice in Stratford upon Avon. We are an excellent forward thinking practice that have a large and well supported clinical team

You will be supporting the GP's and Nurse Partner in the delivery of nursing care, including diabetes, to our patient population.

Ideally, you will have previous experience of diabetes care as part of chronic disease management within Primary Care. Alternatively, you may have a special interest in diabetes and be looking to quickly develop your skills in this area. We will consider applicants with special interest in other areas of nursing

Experience in managing the type of minor illnesses that present in Primary Care would also be advantageous in your application.

Main duties of the job

To work within our current nursing team to deliver a high standard of clinical care to patients. To assist with our Chronic Disease Clinics. The ideal candidate will have a special interest in Diabetes

Job responsibilities

Primary Duties

Your main role will be to implement and evaluate individual treatment plans for chronic disease patients, particularly those with diabetes

To develop, implement and embed health promotion and well-being programmes

To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health

To provide routine nursing care to patients as required in accordance with clinical based evidence

Experience with COPD / Asthma or an interest in developing skills

To request pathology services as necessary

To process pathology results as required

To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

To maintain accurate clinical records in conjunction with extant legislation

To ensure read codes are used effectively

To carry out routine treatment room duties i.e. cervical smears, childhood immunisations, travel vaccines

To maintain chronic disease registers

To chaperone patients where necessary

To prioritise health issues and intervene appropriately

To support the team in dealing with clinical emergencies

To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

To support the clinical team with all safeguarding matters in accordance with local and national policies

To deliver opportunistic health promotion where appropriate

Maintain personal education update

Secondary responsibilities

Monitor and ensure the safe storage, rotation and disposal of medicaments

Participate in local initiatives, QOF and enhanced services to support service delivery and patient care

Support and participate in shared learning within the practice

Continually review clinical practices, responding to national policies and initiatives where appropriate

Participate in the review of significant and near-miss events applying a structured approach, i.e. root cause analysis (RCA)
