We are looking for an enthusiastic and dedicated Practice Nurse to join our friendly, forward-thinking team at The Paddock Surgery, part of the Dewsbury & Thornhill Primary Care Network (PCN).
This is an exciting opportunity to play a key role in delivering high-quality, patient-centred care within a supportive and innovative environment. Youll work as part of a multidisciplinary team alongside GPs, healthcare assistants, pharmacists, and other allied health professionals to provide a wide range of nursing services to our diverse patient population.
Your role will involve delivering chronic disease management, immunisations and vaccinations, cervical screening, wound care, and health promotion, while supporting patients to better manage their own health.

Main duties of the job
As a Practice Nurse at The Paddock Surgery, you will be responsible for delivering a range of nursing services and supporting the health needs of our patients. Your key duties will include:

Providing chronic disease management (e.g. diabetes, asthma, COPD, hypertension).

Job Purpose
The Practice Nurse will provide and maintain a high standard of nursing care for patients, offering advice, health education, and support to promote better health and wellbeing. Working as part of the multidisciplinary team, the post-holder will manage a range of nursing duties, including chronic disease management, vaccinations, cervical screening, wound care, and health promotion, while contributing to the continuous improvement of clinical services within the practice.

Conduct health checks including blood pressure, ECGs, and lifestyle assessments.

Offer contraceptive advice, sexual health guidance, and participate in womens and mens health clinics.

Provide health education and support self-management of long-term conditions.

Support and deliver public health initiatives, including NHS screening and vaccination campaigns.
