Main duties of the job

Routine treatment room GP nurse duties

Ability to deliver care, without direct supervision

The ability to work collaboratively with the wider multidisciplinary team.

Ability to follow the Nursing process-Assessment/Diagnosis/Planning/Implementation/Evaluation

Maintaining the Client's dignity and respect at all times

The Chapeltown Family Surgery is a long established training practice working for the communities of Chapeltown and Harehills. Its strength lies in its size with a small closely knit clinical team who have been working together for many years. Decisions are made democratically with everyones contributions invited and good ideas valued. Job description

Job responsibilities

Key Responsibilities:

Assess, plan, implement and evaluate nursing care for patients presenting with a wide range of conditions within the practice setting

Undertake patient health assessments, including health promotion and disease prevention, and provide health education and advice to patients and their families

Administer vaccinations, immunisations, and travel health advice in accordance with best practice guidelines

Help manage chronic disease clinics, including asthma, diabetes and hypertension, and provide ongoing support and advice to patients

Work closely with the practice team to ensure that clinical governance and risk management processes are followed and adhered to

Maintain accurate and timely records in accordance with the practice's policies and procedures, and ensure that confidentiality is maintained at all times

Work collaboratively with other healthcare professionals, including GPs, Practice Nurses, Healthcare Assistants, and Community Nurses, to provide the best possible care to patients

Undertake additional duties as required by the Practice Manager or Senior Nurse, including participation in clinical audits and training and development activities

Qualifications

Essential

  • Registered Nurse with a valid NMC PIN

Desirable

  • GP practice or community experience
  • Demonstrate an understanding of practice processes and change management
  • Cytology
  • Local referral routes.
  • Networking skills
  • Languages relevant to our community such as urdu, punjabi or bengali

Experience

Essential

  • Working together as part of a mutually supportive team
  • The ability to work under pressure and within your limits
  • Knowledge of clinical governance and risk management processes
  • Excellent communication and interpersonal skills, with the ability to build effective relationships with patients and colleagues
  • Good organisational and time management skills, with the ability to work under pressure and manage competing demands
  • Flexible and adaptable approach to work, with the ability to work autonomously and as part of a team
  • Chronic disease management especially COPD, diabetes, asthma.
  • Assessing, reviewing and developing protocols.
  • High level of clinical skills relevant to the role. Vaccinations, phlebotomy, wound care,
  • An ability to command the respect and trust of your peers.
