Job summary
As the Lead GP for the Care Home Team, you will spearhead
the delivery of high-quality, person-centred healthcare to residents across
multiple care homes. This pivotal role combines clinical excellence with
strategic leadership, ensuring continuity of care, proactive health management,
and collaborative working with multidisciplinary teams.
Youll be
responsible for
Leading clinical decision-making and setting
standards of care across the care home network.
Coordinating with nurses, pharmacists, social
care professionals, and allied health staff to deliver integrated care.
Driving improvements in chronic disease
management, end-of-life care, and medication optimisation.
Championing preventative care and reducing avoidable
hospital admissions.
Mentoring junior clinicians and shaping the
future of community-based elderly care.
Main duties of the job

Clinically Lead and manage the Enhanced
Service in Care Homes Team.

Ensure home rounds are undertaken in line
with national guidance for enhanced health in care -homes i.e. regular
reviews of all residents, structured medication reviews, care plans etc.

Adopt a multi-disciplinary team approach
providing clinical leadership to GP/ACP/Nurse, pharmacy and GPA roles

Work in close liaison with the Care
Co-Ordinators team

Ensure MDT Meetings take place with external
stakeholders including Compton, LD team, Brooklands psychiatry

Maintain Good communication and creating
forums for relationship building and feedback with the care homes

Ensure Internal meetings take place
including:
o
Monthly Team meetings
o
Monthly Workforce planning
o
Weekly Team led Education sessions (Identifying
training and learning needs within the team)
o
6 monthly Clinician 1-1 reviews

Quality improvement with a solution-based
approach to problems, SOP development, implementation and review

Leading complex clinical scenarios including
chronic health problems, complaints, issues around capacity and IMCA
involvement
