Job summary

This is a 30 hour per week role, felxibly PCN Hours Mon - Sun 8am - 8pm.

An opportunity has arisen to join a dynamic team within Wolverhampton South-East Collaborative PCN and Health and Beyond. The post holder will provide clinical services for the Enhanced Health in Care Homes Service and be responsible for ensuring consistent, high-quality person-centred care is delivered to patients in our organisations nursing homes.

The postholder will be a driven, enthusiastic and experienced Advanced Clinical Practitioner (ACP), they must be a skilled Registered Nurse, Registered Pharmacist or Registered Allied Health Professional such as Paramedic, Physiotherapist, Dietitian or Podiatrist with the appropriate attitude, skills and knowledge.

The ACP will use advanced autonomous clinical skills and a broad and in-depth theoretical knowledge base to assess the individual patients needs, formulating a diagnosis, appropriate holistic evidence-based treatment, including independent prescribing and proactively identify, implement and evaluate patient care and treatment, working closely with the team and partners to ensure that a robust management plan is in place for all patients.

Main duties of the job

Work as part of a multi-disciplinary team to deliver safe and high quality care.

Carries out clinical practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.

Delivery of care in line with NSF, NICE guidelines and evidence-based care

  • Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.

Co-ordinate the planning and delivery of care ensuring patients and carers/relatives are involved.

Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.

Diagnose and manage acute and chronic conditions, including drug and non-drug based treatment methods into a management plan.

Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.

Assess, diagnose and treat patients in the care home who require medical attention and/or refer to appropriate agency.

They will work closely with care home providers to ensure high standards of care and support the wider organisation in delivery of all required KPIs aligned to Primary Care.

The Advanced Clinical Practitioner (ACP) will act autonomously within their professional scope of practice providing care for patients face to face and with telephone consultations managing weekly ward rounds.

The postholder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.

The Advanced Clinical Practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.

The postholder will use skills, knowledge and competencies as a qualified Advanced Clinical Practitioner in order to be responsible and accountable for managing patient caseloads for treatments, referral, admission and discharging patients as appropriate.

To clinically manage home rounds with the team, being the point of contact for clinical enquiries and case management for aligned homes

To ensure patient outcomes are at the forefront of care, incorporating Primary Care goals are met and campaigns and vaccinations are supported.

To ensure teamwork and good communication within the team at all times

Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate.

Act as a mentor for junior staff, assessing competence against set standards.
