Job summary

The post holder will provide a first point of contact in the surgery for clinical queries. They will undertake urgent same day clinics and assess, diagnose, plan, implement and evaluate treatment. The post holder will also participate with ward rounds as part of our Care Home service.

They will ensure a high standard of clinical practice is maintained by ensuring care is based on current best practice, embarking upon further training if required and continuing to update clinical knowledge.

Job responsibilities

Job purpose/summary:

The post holder will provide a first point of contact in the surgery for clinical queries. They will undertake urgent same day clinics and assess, diagnose, plan, implement and evaluate treatment. The post holder will also participate with ward rounds as part of our Care Home service.

They will ensure a high standard of clinical practice is maintained by ensuring care is based on current best practice, embarking upon further training if required and continuing to update clinical knowledge.

Job Responsibilities:

Carries out clinical practice at an advanced level using expert clinical skills, critical judgement and knowledge to deliver appropriate and safe care.

Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medicines) or refer to other members of the team.

Ward rounds for the Care Home service.

Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols within the scope of qualified practice.

Advise patients on general healthcare and promote self-management where appropriate.

Perform specialist health checks and reviews in line with national guidance.

Perform investigatory procedures as required.

Undertake the collection of pathological samples including intravenous blood samples and swabs in line with the scope of the practice and national guidelines.

Support the delivery of anticipatory care plans and lead certain community services (e.g. monitoring blood pressure and diabetes).

To communicate to all levels across organisations ensuring that that an effective, person centred service is delivered.

Communicate proactively and efficiently with all colleagues.

Assess, diagnose, plan, implement and evaluate treatment/interventions for patients with complex needs and passed on to the team.

Maintain accurate Health Records, ensuring completion of all necessary documentation associated with patient health care and registration with the practice.

Communicate effectively with patients, and where appropriate, family members and their carers.

Initiate and interpret tests and investigations as appropriate.

Undertake home visits and advanced assessment of house bound patients, resulting in an end to end episode of care through treatment planning and prescribing (as required and appropriate within the scope of clinical competence and practice)

Refers patients to other members of the multi-disciplinary practice teams, secondary care organisations and other organisations as necessary.
