Job summary

You will work as part of the primary care pharmacy team with a working day made up of a variety of different components, including face to face appointments, telephone appointments, email/electronic consultation queries, and medicines reconciliation work.

Main duties of the job

  • Undertaking structured medication reviews for patients with complex polypharmacy and/or multiple long-term conditions.
  • Managing and optimising repeat prescribing processes, ensuring patient safety.
  • Providing specialist advice and support to GPs, nurses, and other healthcare professionals on medication-related issues.
  • Developing and implementing prescribing policies and guidelines in line with local and national recommendations.
  • Contributing to Long Term Condition management clinics (e.g., diabetes, hypertension, asthma).
  • Supporting the practice in achieving QOF & IIF targets and other quality initiatives related to prescribing.
  • Educating patients on their medicines, promoting self-management and adherence.
  • Participating in clinical audit and service improvement projects.

About us

At Falkland we are a forward thinking training practice who pride ourselves on adapting to the ever changing landscape of the NHS. We developed the primary care pharmacist role back in 2016 well before the ARRS scheme. We value the expertise in medicine management and the focussed approach which pharmacists bring to the primary care team. Since then we have built our primary care pharmacy team to 2 clinical pharmacists and 1 pharmacy technician, supported by patient services advisers who have additional training in prescription processes.

We are proud of being a mutually supportive team across the different groups of clinicians and non-clinicians, helping each other to deliver the best possible patient care.

Person Specification

Qualifications

Essential

  • Masters of Pharmacy MPharm Degree or equivalent.
  • Registered with the General Pharmaceutical Council GPhC with an active registration.
  • Independent Prescriber qualification is essential.
  • Minimum of 2 years' experience working as a Clinical Pharmacist, ideally at least one of those within a primary care or GP practice setting.
  • Demonstrable experience in conducting medication reviews and optimising prescribing.
  • Excellent clinical knowledge and understanding of common Long Term Conditions
  • Strong communication, interpersonal, and consultation skills.
  • Ability to work autonomously and as part of a team.
  • Proficiency in using clinical IT systems (e.g., EMIS Web, Docman).

Desirable

  • Experience with clinical audit and service improvement.
  • Background in Long Term Condition management.
  • Experience in supervising or mentoring other team members
