There will also be opportunities to support GP trainees, medical students and other learners within the organisation.
First 4 Health Group has invested significantly in digital systems and innovative ways of working. Clinicians are supported to use EMIS Web, electronic prescribing, digital consultation tools, workflow systems, document management systems and population health management tools to improve patient access and service delivery.
The successful applicant will be expected to use EMIS Web effectively, maintain accurate and timely patient records, use appropriate coding and templates, action pathology results and incoming clinical correspondence promptly, and support the effective use of digital tools across the organisation.
You will also work collaboratively with local Primary Care Networks, community services, social care, mental health teams, voluntary sector organisations and secondary care providers to ensure joined-up and seamless care for patients.

You may be required to work at different First 4 Health Group sites depending on service needs, rotas and cross-cover arrangements. This includes supporting services at Stratford Village Surgery (SVS), Church Road Health (CRH), E7 Health (E7H) and Glen Road Medical Centre (GRMC).
Key responsibilities include providing high-quality consultations for patients with acute and long-term conditions, delivering safe and evidence-based care, undertaking face-to-face, telephone and digital consultations, managing pathology results, prescriptions, referrals and clinical correspondence, supporting chronic disease management and preventative healthcare, working collaboratively with multidisciplinary colleagues, contributing to quality improvement and patient safety initiatives, supporting safeguarding processes for children and vulnerable adults, maintaining high-quality clinical records using EMIS Web, participating in meetings and training, supporting the achievement of QOF, PCN and contractual targets, contributing to innovative and digital models of care and supporting patients across all First 4 Health Group sites including SVS, CRH, E7H and GRMC.

You will be expected to work flexibly across clinical sessions and provide support for same-day demand, routine appointments, enhanced access sessions and on-call arrangements where required. You will contribute to safeguarding discussions, multidisciplinary team meetings, care planning for vulnerable patients and end of life care discussions. You will also support the development of junior staff, provide informal supervision where appropriate and contribute to maintaining a positive, inclusive and respectful working culture across the organisation.
