We are pleased to release this exciting opportunity for an experienced nurse to take on a position ideal for someone to join us from either a Practice base or direct from a hospital to join our Practice and complement our existing nursing team. We are a hardworking, rapidly moving, pro-active, high achieving Practice with two sites on the A64 York Road.

We are looking for a candidate who currently holds, or is prepared to train towards, the relevant qualifications to be able to deal with chronic disease management, wound dressings, cytology, child immunisations and all other areas of modern nursing.

You will be able to work autonomously demonstrating forward thinking and skills in clinical decision making when managing patients and your role is integral to contributing to the safe and smooth running of the practice.

The successful applicant will be expected to demonstrate excellent patient care. You will be able to provide the provision of preventative health care for patients along with reporting in accordance to the Care Quality Commission within the practice.

Knowledge of SystmOne clinical system would be an advantage but not essential and an understanding of QOF would be a distinct advantage.

Treatment Room Role: Carry out new patient assessments, Wound management, suture removal, Venepuncture, vaccines and imms, blood pressure, Cytology, Spirometry, weight, height, BMI, Chaperoning and home visits.

Chronic Disease Management: Contribute to the management of patients with chronic diseases, identified by QOF in particular, Asthma, COPD, CHD, Depression and Diabetes. Learning Disabilities and Mental Health.

Health Promotion: Address patient in order to identify problems of: Diet, Exercise, Smoking Cessation, Obesity, Alcohol, Drug misuse, Family Planning, the Menopause and HRT.

Health Screening : Organisation and management of health screening activities for patient Immunisations, Cervical cytology, Well person checks and Child health & Child Protection

Infection Control: Adopt infection control measures within the Practice according to the Infection Control Manual.

Administrative and professional responsibilities: Ensure accurate notes associated with patient health care are recorded in the patients notes on the clinical system.

Attend Practice meetings and participate in an annual performance review of your own personal and professional development.

BASED AT: York Road and Rookwood Avenue.

WORKING HOURS: Your hours of work will be dictated by the requirements of the surgery, but your usual working pattern will be 08.30am to 12.30pm, 2.00pm to 6.00pm, during some days between Monday to Friday with a one and a half hour lunch break.

PURPOSE OF YOUR JOB: To work as part of the primary healthcare team providing care to practice patients to include treatment, chronic disease management, preventative care, screening and patient education in all health promotion matters. The post holder will be able to assess and plan the nursing care of individual patients, delivering that care appropriately according to practice policies, protocols, and National Service Frameworks etc, to the highest standards. The job holder will also be able to document due processes of assessment of need, delivery and outcomes of such care, using the clinical system to its fullest extent. Finally, the post holder will refer patients / concerns, as appropriate, to GPs for good patient management.

PRINCIPAL ACTIVITIES:

a. Treatment Room:

To carry out new patient assessments, incorporating an holistic approach to care which will help identify individual needs in relation to their physical, mental and social well being.

To provide general nursing care for patients in accordance with practice guidelines which include the following:

Wound management

Removal of sutures

Venepuncture

Dealing with minor injuries

Administration of medicines

Ensure safe storage, rotation and disposal of vaccines and drugs

Immunisations & Vaccinations childhood, flu, shingles etc.

Screening activities, eg. lipids, BP

Cervical Cytology

Spirometry testing

Weight, height, BMI

Emergency treatment - Competency in anaphylaxis and resuscitation

Management of medical supplies stock and travel vaccines

Chaperoning and assisting patients who are being examined by another clinician.

To take responsibility for infection control issues.

Ensure maintenance of treatment room equipment in participation with the practice manager.

Monitor and maintain stock levels.

Maintain and clean equipment used by nurses and GPs.

Maintain general tidiness and cleanliness of nurses and treatment rooms.

On occasions which dictate it, there may be the need to undertake home visits.

b. Chronic Disease Management

As part of a multidisciplinary team, contribute to the management of those patients with chronic disease, as identified by the Quality Outcomes Framework, in particular, Asthma

COPD, CHD, Depression and Diabetes.

Manage specific clinic sessions as required.

Maintain an appointment and recall system for patients.

Mental Health

c. Health Promotion

Address the health promotion needs of the practice population taking a reactive, opportunistic and proactive approach in order to identify potential problems in the following areas:

Diet and Exercise Advice

Smoking Cessation

Obesity

Alcohol

Drug misuse

Management of Family Planning Services in conjunction with the GP, to include contraceptive provision, advice and education to include the Menopause and HRT.

Develop & maintain health Promotion material within the practice and waiting rooms.

d. Health Screening

Organisation and management of health screening activities in the following areas:

Child Immunisations

Womens health include Cervical cytology

Mens /Young peoples health

New patient / Well person checks

Child health & Child Protection

e. Infection Control

Support patients and colleagues in adopting sound infection control measures

Apply infection control measures within the practice according to the Infection Control Manual provided by the CCG, working to local and national guidelines, including universal hygiene precautions, collection and handling of specimens, segregation and disposal of waste materials, reporting and treatment of sharps injuries, and dealing with blood and body fluid spillages.

f. Administrative and professional responsibilities:

Participate in the administrative and professional responsibilities of the practice team.

Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical system as appropriate.

Attend and participate in practice meetings.

Participate in an annual individual performance review, including taking responsibility for maintaining a record of your own personal and professional development.

Take responsibility for your own development, learning and performance.

Assist in the formulation of practice philosophy, strategy and policy and protocols.
