Job summary
We are looking for a compassionate and enthusiastic Practice Nurse to join our friendly and supportive clinical team.
We are a well-established practice serving around 6,200 patients, with an excellent reputation for high-quality, personalised, patient-centred care. Continuity of care and the culture of traditional family medicine are strong values in the practice, and support our "cradle to grave" relationship with patients.
We would prefer applications from applicants who have previous experience of working in a GP practice, but we would also welcome applications from nurses with a background in Community Nursing.
The role will cover all aspects of practice nursing, including:
- Baby immunisations
- Cervical screening
- Long-term condition reviews
- Travel vaccinations
- Wound care / ulcer care / doppler

Main duties of the job
Provide safe, evidence-based nursing care to patients of all ages.
Carry out baby and childhood immunisations in line with NHS vaccination schedules.
Manage patients with chronic illnesses such as diabetes, COPD and asthma.
Undertake long-term condition reviews, including health checks and lifestyle advice.
Contribute to meeting QOF and Enhanced Services targets.
Maintain accurate and timely patient records using clinical IT systems (e.g. EMIS).
