SystmOne, Accurx Total
Triage and Ardens Template embedded practice.

We are a 3 site GP
practice with a single M-number and would expect the post-holder to work across
all 3 sites.

A team
of 7 Partners and 10 Salaried GPs as well as 5 Paramedics (who cover the
majority of home visits), a team of Clinical Pharmacist and a very supportive
team of Nurses, HCAs and Phlebotomists

excellent skilled admin team and highly supportive management team working
across all three sites

Is
part of a forward thinking, supportive PCN with a well established and rapidly
growing clinical team

Typical clinic includes
14 patients per clinic with a small amount of clinical admin.

Majority of prescriptions
are managed by our prescribing team along with much of our docman being managed
by our workflow team.

It is rarely required to
do a home visit as these are typically undertaken by our paramedics.

We are a clinician led
total triage practice and allocate the Oncall and triage sessions equally and
fairly amongst our GP workforce.

Main duties of the job
We offer lots of learning
opportunities within the practice and have multiple GPs with extended roles
(including Dermatology, MSK, Diabetes, Womens health) and would be happy to
support and encourage GPs to pursue their interests.
We are a teaching
practice and have 3rd and 4th year UoB medical Students
along with GP Registrar trainees.Lordswood is also a
registered research practice undertaking studies with NIHR

Job responsibilities
In accordance with the
Practice timetable, as agreed, the post-holder will be available to undertake a
variety of duties including surgery consultations, telephone consultations,
visiting patients at home, checking and signing repeat prescriptions and dealing
with queries, paperwork, reports and correspondence in a timely fashion
Triage patients presenting
to the surgery by telephone, online and in person
Making professional,
autonomous decisions in relation to presenting problems, whether self-referred
or referred from other health care workers within the organisation
Assessing the health care
needs of patients with undifferentiated and undiagnosed problems
Screening patients for
disease risk factors and early signs of illness
In consultation with patients
and in line with current Practice disease management protocols, developing care
plans for health and promoting healthy lifestyle strategies and education
Manage long-term conditions
and patients with complex problems
Recording clear and
contemporaneous consultation notes to agreed standards
Collecting data for audit
purposes and complying with QOF requirements
Delivering enhanced
services
Check, manage & process
patient test results
Refer patients to secondary
care and/or relevant organisations and internally within the PCN (eg to
additional roles members of the PCN team) as required
Compiling and issuing
computer-generated acute and repeat prescriptions (avoiding hand-written
prescriptions whenever possible) in line with local and national guidelines
Prescribing in accordance
with the Practice prescribing formulary (or generically) whenever this is
clinically appropriate
In general the post-holder
