motivated and forward-thinking Practice Nurse to join our innovative,
training-focused practice serving 20,000 patients in Wallington.
The Practice is committed to
delivering high quality patient care which requires us to have a supportive and
cohesive team.
We require a skilled and
enthusiastic nurse with chronic disease experience to join our nursing team.
The position is available for an immediate start.
Our team benefits from a wide
range of clinical expertise; experience in Respiratory, and Diabetes management
would be particularly valuable.

Main duties of the job
The practice nurse will work as part of the primary
healthcare team providing Medical Services to the patients of the practice.
The post holder will be responsible for the
delivery of nursing services, working as part of the practice multidisciplinary
team, delivering care within their scope of practice to our patient
population. The practice nurse will be responsible for a number of clinical
areas such as health promotion, chronic disease management, health prevention,
well women and well man clinics, as well as supporting the management team in
the reviewing of clinical policy and procedure.
About us
Wallington Family Practice currently provides general medical services to
a population of approximately 20,000 patients. It has 11 GPs, A Team of
Pharmacists including Pharmacy Technicians, a Lead Nurse, 5 Practice Nurses, 1
HCA, and an extensive team of admin and reception staff.
We are a large proactive teaching practice located in a purpose built
health centre close to Wallington town centre. We are part of the Wallington
Primary Care Network (PCN) working collaboratively with local general practice
colleagues for the good of the local population.
The
main purpose of this role is to provide efficient, courteous and responsive
care navigation to the diverse needs of our patients

Main Clinical Duties
- According to qualifications and experience, to work as an autonomous practitioner within the multi-disciplinary team to provide a full range of Practice Nursing services, including:
- Cervical Cytology
- Chronic Disease Management including Asthma, Diabetes, Coronary Heart Disease, COPD (when qualified and trained to do so)
- Patient Education and Health Promotion
- Wound management
- Ear irrigation
- Contraception advice
- Vaccinations and immunisations
- Travel health vaccination and advice
- Other general treatment room services as required
- According to qualifications and experience to provide assessment, implementation and evaluation of clinical packages of care.
- Provide health education and work with the patient and other healthcare professionals to effectively deliver appropriate packages of care
- According to qualifications and experience, advise patients and other clinical staff on the management of chronic diseases and appropriate therapies and pharmacological intervention
- Refer patients independently to other clinical disciplines internally and externally as appropriate for further care
- Undertake clinical audit activities to review care provided, make recommendations for improvement in care and implement agreed changes
- Assist with QOF and other target work to ensure maximum performance for the practice
Administrative duties
- Ensure that accurate and contemporaneous notes of consultations are kept on the computer system (currently EMIS Web).
- Assist in the maintenance and monitoring of medical supplies and also the ordering and recording of medical supplies from appropriate suppliers and in accordance with normal practice procedures. Ensure appropriate storage on receipt of supplies and equipment
- Ensure regular routine servicing and maintenance of medical equipment and maintain record of these accordingly including details of items for repair.
- Ensure that clinical rooms are well stocked and maintained.
- Assist with any recall or follow-up systems required to encourage patient attendance.
Communication
- Establish and maintain effective working relationships with all members of the clinical and non-clinical team including regular attendance at nursing team meetings and other appropriate meetings within the practice, the locality and CCG as a whole.
- Communicate effectively with patients and their relatives through verbal and non-verbal means.
- Assess the patients ability to give informed consent, and request assistance from another clinician if unsure.
- Read and reply to internal and external emails on a regular basis as a means to communicate with other staff members.
Confidentiality and Data Protection
The post holder will have access to sensitive information about patients health and other matters. It is imperative that staff respect patients privacy, confidentiality and act appropriately.
Information must be disclosed to an appropriate clinician if it is felt that the patient were at risk of harm to themselves or others in accordance with the Practices policies.

Ensure mandatory training is up to date and understanding that this training may need to be undertaken on days or times not normally worked.
