We are seeking an experienced and qualified Advanced Practice Nurse with prescribing abilities and a special interest in diabetes. In this role, you will play a pivotal part in delivering exceptional clinical care to our practice population, working autonomously within a multidisciplinary team.
Main duties of the job
About the Role: As a Specialist Diabetic Practice Nurse, you will:
- Provide advanced care for patients with uncontrolled and complex diabetes.
- Identify, manage, and support patients at risk of or with poorly controlled diabetes.
- Develop and implement care plans, prescribe within your competence, and support junior staff.
- Deliver health education, audit care standards, and contribute to service development.
- Undertake occasional home visits and liaise with secondary care colleagues and agencies.
