Job Purpose

To provide a high quality, easily accessible nursing service to the practice population, making best use of clinical skills with a focus on diabetes, within the practices enabling the provision of a diverse range of services within the competence of the post holder. Working at an advance level and focusing on the prevention, management, and treatment of Diabetes.

General Requirements

To provide clinical care to the practice population registered with Primary Care Sheffield Practices.

To work autonomously as part of the multidisciplinary team, managing own workload, to include assessment, development, implementation, and evaluation of care in complex patients.

To be aware of confidentiality issues and practice these in accordance.

To ensure a safe working environment fulfilling the requirements of the Health and Safety Policy.

To be sensitive to the needs of the multicultural community.

To be willing to extend personal knowledge and ability by continuing education both within the practice and by outside agencies.

To take responsibility for personal competency including participation in clinical supervision.

To work within the NMC code of professional practice.

Responsible for continuing personal development in line with Primary Care Sheffield Group policies.

To have an annual PDR with line Manager.

To undertake further training as required enabling duties to be carried out.

Principal Duties

Delivery of clinical care and health education to the practice population.

Provision of clinical services in Diabetes, including advance care of uncontrolled, complex patients.

Prescribing within clinical competence.

Identifying, managing, and supporting patients with poorly controlled diabetes or at risk of diabetes.

Have formal qualifications in Diabetes and a proven history of working with diabetic patients.

Have a special interest in Diabetes and to keep up to date with these skills.

Willing to undertake future training in complex long term conditions, such as dementia.

Working at an Advanced level. Reviewing and acting on blood results for long term conditions. Initiating care from these results and prescribing accordingly.

Creating patient care plans and supporting junior staff in implementing these plans.

Providing educational and counselling support to patients and junior staff members.

Using internal, local and national policies, Assess, plan and organise patient care, including evaluation of need, adjustment of care plans and recall.

Audit of care provided, aiming for a high standard of care and achieving targets for Quality Outcomes Framework.

Using and implementing research in the practice to ensure up to date clinical care is given and development of services.

Working to continually improve QOF data.

To maintain accurate and contemporaneous records.

Planning and organisation of clinics in discussion with other team members.

To liaise and communicate with other team members, secondary care colleagues and other agencies where appropriate.

To educate other team members both clinical and administrative, as appropriate.

To be involved in the training and development of student nurses whilst on placement.

Willing to undertake occasion home visits if required.

To provide nursing leadership at sites, including undertaking PRDs for staff.

Service Development

Assist in development of practice policies and procedures.

To ensure care is delivered with regards to evidence based practice and the implementation of national and local guidelines.

Communication and Interpersonal Skills

Essential

• Excellent written and verbal communication skills
• Experience in liaising with General Practitioners and other Health Care Professionals

Clinical Knowledge and Skills

Essential

• Competent clinical skills with a strong knowledge of chronic disease management.
• Able to use analytical skills in practice.
• Able to undertake Audit and implement changes from audit.
• Able to undertake quality improvement projects.
• Undertake or be part of research.
• Excellent interpersonal and organisation skills.
• Clear understanding of legal issues, professional accountability and responsibility.
• IT experience.

Desirable

• Training in understanding blood results.

Qualifications

Essential

• RGN
• V300 Non-Medical Prescriber
• Diploma or equivalent in Diabetes
• Mentor Assessor Qualification

Desirable

• Chronic disease diploma or equivalent in Asthma, COPD, Coronary Heart Disease, Chronic Kidney Disease, Stroke.

Experience

Essential

• 5+ year practicing as a Practice Nurse in Primary Care or Working as a diabetic specialist nurse or equivalent in Primary Care.
• Experience of multi-disciplinary working.
• Able to demonstrate high level of personal professional autonomy and accountability.
• Use evidence-based knowledge, professional and clinical judgement to assess, plan and implement high-quality person-centred care.
• Management of complex care.
• Experience in leadership or line management.

Management

Essential

• Experience in managing and mentoring junior staff members, practice nurses, NAs, TNAs, HCA and student nurses.
• Ability to be flexible and adaptable to change.
• Understanding of clinical governance.

Self Management

Essential

• Ability to work under pressure.
• Able to manage time effectively.
