Job summary

The post holder will be an experienced ANP/ACP who is already established in the role, demonstrating clinical competence and critical thinking. They will be a caring, compassionate and committed practitioner who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment and diagnosis, through to treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care for patients within the general practice population. They will be able to demonstrate excellent communication skills and work collaboratively with the general practice team to meet the needs of patients, as well as autonomously, supporting the delivery of policy and procedures, and providing clinical leadership as required.

Main duties of the job

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis acute & chronic.

Clinically examine & assess patient needs from a physiological & psychological perspective, & plan clinical care accordingly.

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition.

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs both in accordance with evidence-based practice and national/practice protocols, within scope of practice.

Work with patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication including medication regimens, side-effects and interactions.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency treatment.

Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.

Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.

Be competent in dealing with both paediatric and adult presentations

To perform Home visits and/or Cares Homes as required.

About us

Practice has been in existence for in excess of 50 years. The Practice was previously located in a building near Slade Hall before relocating to current facility.

West Point Medical Centre is both a Teaching and Enhanced Training Practice. We teach Year 4 and Year 5 Medical Undergraduate Students, usually placed by the University of Manchester, as well as Nursing and other Healthcare Students. The Practice also trains doctors who are in their final three years of GP training.

Our 3 partners, Dr Askew and Dr Hannan and ANP Matthew Johnson, hold a General Medical Services (GMS) contract with NHS England to provide NHS services to patients registered with the Practice.

As such, West Point Medical Centre is NOT a NHS body. The Partners, Dr Askew, Dr Hannan ANP Matthew Johnsonare therefore the business owners, as well as practicing GPs and ANP.

The Practices members of staff are therefore employed by West Point Medical Centre and NOT the NHS although staff do enjoy some NHS benefits such as membership of the NHS Pension Scheme.
