We are looking to recruit a motivated, committed and enthusiastic, Advanced Clinical Practitioner to join our friendly general practice with a patient population of around 6000. The post is for 37.5 hours per week, flexible working.

Clinical staff includes three GP Partners, three salaried GPs,Practice Nurse ,Healthcare assistant and GP Trainee/s. This is a training practice.

We are looking for a highly motivated Advanced Clinical Practitioner with excellent communication and interpersonal skills. You will be responsible for ensuring the delivery of safe and effective care to the whole practice population, working closely with the practice manager and GPs to deliver the practice priorities.

Applicants must be flexible and a team player. This position is for 37.5 Hours a week, working over five days with time commitment to the needs of the surgery and will be variable.

Main duties of the job

To be responsible for the delivery of good quality General Practice Nursing to the practice population.

The focus of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition, and the provision of preventative health care to the practice population. As an autonomous practitioner the ACP is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the General Practice team to meet the needs of patients, support the delivery of policy and procedures, and provide ACP leadership as required.

Good communication is extremely important within the Practice, with patients and with outside agencies and you will be expected to help ensure that everyone who comes into contact with it holds the Practice in high regard. Absolute discretion is required in protecting the patients confidentiality.

About us

The GP Practice is located in Stalybridge City Centre close to the bus and train station. We are housed in a large Victorian building with plenty of accommodation. We are part of Stalybridge, Mossley and Dukinfield Primary Care Network and the Network hub is situated on the first floor of the surgery building.
