Job summary

The Linden Medical Group is seeking a motivated and organised QOF & Practice Administrator to join our friendly and supportive primary care team.

This role combines Quality and Outcomes Framework (QOF) administration with general practice administrative duties. The successful candidate will play a key role in supporting clinical teams, maintaining accurate patient records, managing recall systems, and ensuring the practice meets contractual and governance requirements.

Main duties of the job

  • Manage and maintain QOF and clinical data using the SystmOne clinical system.

  • Monitor QOF indicators, generate reports, and support the practice in achieving QOF targets.

  • Ensure all eligible patients are recalled for annual QOF and chronic disease reviews.

  • Manage non-QOF recalls, including DOAC monitoring and secondary careinitiated recalls.

  • Process clinical correspondence and pathology results in line with practice procedures.

  • Download, attach, code, and forward clinical letters to the appropriate clinicians.

  • Use MJOG SMS and other systems to invite patients for reviews and appointments.

  • Support audits, inspections, and reporting requirements.

  • Work closely with the Practice Manager and clinical team to improve performance and patient outcomes.

  • Ensure administrative SOPs are followed and kept up to date.

About the Practice

The Linden Medical Group is a well-established, forward-thinking GP practice committed to delivering high-quality patient care within a supportive and inclusive environment.

Our clinical team consists of three GP Partners, two Salaried GPs, two Practice Nurses, one Nurse Practitioner, one GP Assistant (GPA), and one Healthcare Assistant (HCA). This is supported by a dedicated and experienced administrative and reception team, who play a vital role in the day-to-day running of the practice.

We also work closely with a range of attached multidisciplinary team members, including clinical pharmacists and a Mental Health Occupational Therapist, enabling us to provide comprehensive, patient-centred care.

Key ResponsibilitiesQOF and Data Management

  • Maintain accurate, complete, and up-to-date patient records using the SystmOne clinical system.

  • Input, review, validate, and code QOF-related clinical data in line with national and contractual requirements.

  • Generate, analyse, and interpret QOF and performance reports to identify gaps and areas for improvement.

  • Attach letters to patient records, apply appropriate clinical coding, and forward to the relevant clinician.

  • Distribute incoming pathology results in accordance with practice protocols.

Reporting, Audit, and Compliance
  • Prepare and submit QOF progress and performance reports for internal review and external audit purposes.

  • Support clinical audits, inspections, and contractual reviews as required.

  • Keep administrative Standard Operating Procedures (SOPs) accurate and up to date.

  • Remain up to date with changes to QOF, contractual requirements, and NHS guidance, ensuring the practice remains compliant.

  • Work closely with the Practice Manager to develop and implement strategies to maximise QOF achievement.

  • Regularly review performance data with the Practice Manager and highlight risks or underperformance.

  • Provide administrative and data support to clinicians and the wider practice team.
