Job summary
We have an exciting opportunity for an experienced and enthusiastic Practice Nurse to join our multi-disciplinary primary care team to improve the health and wellbeing of our patients .
Although working independently, you will work alongside our GPs, Pharmacists and Nursing Team. The succesful candidate will provide all teatment room duties and support with CQC compliance.
This is a permanent position up to 32 hours. In addition you will be required to take part in the Extended Access rota on Saturday mornings and annual vaccination clinics.
Main duties of the job
To provide nursing services to a defined practice population
with the assistance of, and in collaboration with, the Nursing Team and GP
colleagues. Clinical duties to include: ECGs, wound care (leg ulcer level 1),
BPs, Ear irrigation, Injections, cervical screening, chronic disease management
and care planning and other clinical duties as requested within capability. To
contribute in the development of services provided by the Practice and to
participate in clinical audit. To work
with the GPs to develop and implement the Practice Development Plan, and to
meet key performance indicators within Quality and Outcomes Framework (QOF) and
Enhanced Commissioning Framework (ECF). You will be expected to work within he
NMC Code of Conduct and to agreed practice standards and protocols.

The Garden City Practices is committed to development of its staff and is an accredited training practice. Our clinical team consists of 6 GP partners, 2 ANPs and a Nursing Associate as well as clinical support via the PCN Additional Roles Reimbursement Scheme (ARRS)
We are based 5 minutes walk for the town centre, with onsite parking, and are an active member of the WGC PCN with our neighbouring practices and part of the local Federation, Ephedra.
There is an experienced, friendly team of administrative staff including the Practice Manager, Deputy Practice Manager, Reception Manager & Team, Senior Administrator and Secretaries.
Our list size is growing and currently we have around 12 500 patients.

- To work with the Nursing team to assess, plan and provide health care and nursing services to the practice population, ensuring they are effective, relevant, evidence-based and of a high standard
- To work with the Nursing team to provide a comprehensive general practice nursing service, to include wound care, urine tests, childhood immunisations, seasonal influenza and childhood nasal influenza, pneumonia, shingles, rotavirus immunisations, covid -19 vaccinations, NHS assessments, ECGs, blood pressure, travel immunisations and lifestyle and dietary advice following cholesterol and HBA1c blood tests.
- To provide a womens health service, to include cervical screening and advice and information regard breast screening, Chlamydia testing and family planning
- With the Nursing Team and GP colleagues to achieve and maintain cervical screening targets. To record and monitor result, including efficient and accurate call and recall systems.
- To work with the Nursing team to provide an effective health promotion service, to include healthy lifestyle clinics.
- To work with child health, to include the administration of the childrens immunisation programme according to current schedules, the accurate recording of the information in the child health records and to participate in measures (such as education sessions) to improve the uptake of childhood immunisations, especially the Measles, Mumps and Rubella vaccine, whooping cough in pregnancy.
- To work with the practice team to organise, implement and administer the annual influenza and pneumococcal vaccination programme, to include the identification, targeting and immunisation of vulnerable patients within the practice population, to ensure that local and national targets are met.
- To work with the Nursing team to identify and maintain registers of those patients with chronic disease such as asthma, chronic obstructive pulmonary disease (spirometry), diabetes, thyroid disease, epilepsy and coronary heart disease, hypertension.
- To assist in the development, implementation, delivery and evaluation of programmes of care for those patients with chronic disease as dictated by the National Service Frameworks targets and to meet and maintain the quality indicators laid down in the nGMS contract initiated in April 2004
- To ensure that professional responsibilities are met as practitioner supplying and administering medicines in accordance with a Patient Group Directions .
- To assess patient understanding of treatment proposals, gain valid informed consent and works with the NMC Code of Professional Conduct to provide care for patients who lack the capacity to consent to treatment.
- To use a range of verbal and non-verbal communication tools, to communicate effectively with patients to deliver effective healthcare. This will include patients who have special communication needs, e.g., those patients who are sensory impaired or who have mental health issues.
- To work with the practice team to ensure clinical governance standards are met, including managing clinical risk within own clinics.
- To work closely with the practice team, primary care and community nurses, allied health professionals and social services colleagues by active participation in clinical meetings, to ensure effective lines of communication to achieve high standard and continuity of care for the practice population.
- To work with the Nursing team and GP colleagues to develop local enhanced services, if required.
To be professionally accountable and responsible for all aspects of your own work, including the management of patients in your care, always practising in accordance with the NMC Code of Professional Conduct.
To maintain a professional portfolio which reflects personal development and to attend clinical supervision.
To be responsible for maintaining competency to practices through ones own continuous personal and professional development activities, To maintain an annual professional development plan through appraisal, in accordance with the CCG. Performance Development Scheme and that is relevant to the practice development plan.
To undertake the measurement and evaluation of your work using audit (such as cervical smear adequacy), outcome measures and evidence-based practice projects.
