Job summary

Due to expansion of the nursing team we are looking for an experienced Practice Nurse to join our friendly team. The role is for 3 days per week. In addition there will be a requirement to take part on a rota basis on Saturday mornings as part of the practice's commitment to extended hours.

We moved to refurbished premises in 2019 and are within walking distance of Welwyn Garden City town centre and station. There is on-site parking to the rear. You will be part of a nursing team of 2 ANPs, 1 Practice Nurse and a Trainee Nursing Associate.

Main duties of the job

We are looking for an experienced Practice Nurse who can undertake cervical screening, full range of immunisations including childhood imms, travel vaccinations and flu and covid vaccinations, B12 injections, wound care (level 1 leg ulcers may be part of this role) and the full range of treatment room duties. Long term condition management and treatment care plans will also be part of this role.

About us

The Garden City Practice clinical team consists of 6 partners and an excellent nursing team supported by a hardworking and friendly administrative team. We are a training practice and often host medical students and GP Trainees.

We are part of the WGC Primary Care Network with the other local practices, and offer Extended Hours via the local Federation, Ephedra at Spring House.

As part of the PCN we also have Clinical Pharmacists, First Contact Physiotherapists, Mental Health Practitioner and Social Prescribers as part of the team to provide excellent patient services.

Job description

Job responsibilities

To work with the nursing team to provide a comprehensive practice nursing service to include wound care, childhood immunisations, seasonal and childhood influenza programmes, pneumonia, shingles vaccinations, ECGs, NHS Health Checks etc.

To work with the GPs and Nursing Team to achieve cervical screening targets

Manages programmes of care for patients with long term conditions by planning, providing and evaluating care under agreed local/national guidelines

To ensure professional responsibilities are met as practitioner supplying and administering medicines in accordance with Patient Group Directions (PGDs)

To work with the practice team to ensure clinical governance standards are met, including managing clinical risks within own clinics

To participate in clinical and practice meetings

To ensure implementation of infection control and prevention measure within the practice

To contribute to the management of the nursing services and resources, including ordering of stocks, maintaining the cold chain and stock rotation monitoring expiry dates

To ensure accurate patient notes are recorded and by use of templates in SystmOne enter appropriate Read Codes to maximise practice achievement for QoF, CFF and other Enhanced Service Targets
