Community Leg Ulcer Service Nurse

The closing date is 13 February 2026

Job summary

Diamond Health Group provides a Community Leg Ulcer Service across Somerset, receiving referrals from local GP practices. Clinics are delivered at a range of satellite sites, currently located in Langport, Ilminster, Crewkerne, Yeovil and Wincanton.

We are seeking a motivated and enthusiastic Practice Nurse to join our friendly and supportive Community Leg Ulcer Service team. This is an excellent opportunity for a nurse with an interest in wound care and leg ulcer management. A comprehensive training and induction programme will be provided, making this role suitable for nurses who wish to develop specialist skills in this area.

This is a fixed-term, part-time position for a period of 12 months. The successful candidate will work 21 hours per week, with working days being Tuesday, Thursday and Friday.

There is potential for the role to be reviewed and extended at the end of the fixed-term period, subject to business need.

Main duties of the job

The post holder will demonstrate safe, effective clinical decision-making and work with a degree of autonomy, supported by senior clinicians and a structured training programme. Duties will include:

  • Assessing and managing new patient referrals into the Community Leg Ulcer Service
  • Undertaking Doppler assessments (training provided if required)
  • Applying compression therapy and bandaging in line with evidence-based practice
  • Managing and prioritising a clinical caseload, ensuring care is tailored to individual patient needs
  • Maintaining accurate, timely and confidential clinical records
  • Working collaboratively with GPs, the wider nursing team and other healthcare professionals
