We have an opportunity for a General Practice Assistant to join the team at Diamond health Group. We are well-established GP surgery providing patient focused care to approximately 14000 patients across two sites. We are seeking to recruit a General Practice Assistant (GPA) to support the wider practice team with a variety of administration duties. You will be joining our two current General Practice Assistants and you will have the opportunity to complete a GPA skills training program during your employment. The hours for the position is 37 hours per week and the working pattern will be discussed further at interview and confirmed with the successful candidate upon job offer. The successful candidate will be expected to support the practice staff in the smooth running of patient services which can include the handling of routine administration and some basic clinical duties. The job role will be supported with full training.
Main duties of the job
-Extracting information from clinical letters that require coding and adding to the patient notes
-Arranging appointment, referrals and tests for patients
-Supporting QOF Framework with monthly patient recalls for chronic disease management
-Ensure that meeting actions are recorded, disseminated and followed up in a timely way; ensuring that relevant practitioners are aware of meeting decisions and actions/ outcomes, chase for action resolution and update
-Completing basic (non-opinion) forms and core elements of some forms for the GP to approve and sign such as insurance forms, mortgage, benefit agency forms etc
-Support the management team and the senior practice management with project work and basic administration such as HR filing
-Supporting all clinical post and prioritising for the GP in terms of actions. Signposting some post to others such as to the prescription team

Employment is provided by the Yeovil Primary Care Network and is based at Hendford Lodge Medical Centre. The Yeovil Primary Care Network is an NHS collaboration between 5 GP practices; Penn Hill Surgery, Ryalls Park Medical Centre, Preston Grove Medical Centre, Diamond Health Group and Oaklands Surgery.
Primary care networks (PCNs) form a key building block of the NHS long-term plan. Bringing general practices together to work at scale has been a policy priority for some years for a range of reasons, including improving the ability of practices to recruit and retain staff; to manage financial and estates pressures; to provide a wider range of services to patients and to more easily integrate with the wider health and care system.
Diamond Health Group is situated in central Yeovil with a branch site on the outskirts of Abbey Manor Park. We offer 27 days annual leave per year (pro rata) and enrolment into the NHS pension scheme.

Key Responsibilities

Support the PCN and Practice Managers in the delivery of the Network Contract DES and Quality and Outcome Framework specifications.
Ensure that meeting actions are recorded, disseminated, and followed up in a timely way; ensure relevant practitioners are aware of meeting decisions and actions / outcomes, and chase for action resolution and update.
Support with chronic disease and drug monitoring recall systems
Review laboratory results and action according to agreed protocols
