Job summary
The post holder will work within the Homewell frailty team but
will also liaise closely with the clinicians in the practice.
This will have a focus on long-term medical
conditions such as diabetes, hypertension, and cardiovascular disease, but will
also aim to take a wider holistic view, including providing information,
advice, or referrals to other services.
Must be able to drive / have transport. Any fees occurred
i.e. Petrol will be compensated.
Main duties of the job
To visit patients at home as part of the
frailty team.
To obtain specimens for basic investigations
such as blood and urine analysis.
To ensure that such specimens are prepared,
labelled and submitted appropriately for laboratory analysis.
To perform clinical measurements such as
blood pressure, pulse rate, height, weight and BMI.
To obtain simple items of medical history
such as smoking history and alcohol consumption.
To ensure that all the health-related
information collected is documented accurately, clearly, and concisely in
patient electronic records (EMIS) and is made available to other members of the
frailty team.
To inform other members of the frailty team
and the patient's own GP should any matters of concern be identified during
home visits to patients.
To provide appropriate signposting to a
relevant clinician or external organisation (whilst ensuring that the patient
receives appropriate ongoing care).
To participate in patient vaccination
programmes where appropriate (in accordance with level of training).
To support and advise patients on the
promotion of health, prevention of ill-health and the self-management of their
health within their own abilities.
To ensure that all necessary arrangements are
completed promptly and in a safe manner.
To maintain effective communication with GP
practice and other appropriate healthcare teams to the benefit of the patient.
