Job summary
This Advanced Nurse Practitioner role will involve working alongside a
multidisciplinary team, including GPs, Nurse Practitioners, Clinical
Pharmacists and Nurses to provide care and treatment to patients with either
long term conditions or undiagnosed undifferentiated diagnoses.
Main duties of the job
The successful Advanced Nurse Practitioner will work as an autonomous
practitioner, diagnosing, managing, prescribing and discharging patients with
illness or injury across all age groups including young children. This may be
by face-to-face consultation or via telephone call.

for approximately 6200 patients with a practice area that extends across
Newport East. We employ a full range of professional medical and
clerical staff and our large, spacious premises enable us to house
additional services. We have a strong focus on training and development
within the practice and accommodate GP Registrars, nursing students and
administration apprentices.
Based in the heart of Newport, close
to the local bus and train station, there is one waiting area available
for patients, which provide a comfortable waiting space. The practice
building has a first and second floor, with 6 clinical spaces.
With
an exciting move coming in the nearer future, we are looking to expand
our clinical team. Aneurin Bevan University Health Board are planning to
build a new integrated Health and Wellbeing Centre in Newport East. The
Welsh Government has approved the full business case for the new
Newport East Health and Wellbeing Centre development, which is expected
to open in January 2025. The Health & Wellbeing Centre will be based
in Ringland and house two existing GP Practices working independently;
Ringland Medical Practice and it is proposed, Park Surgery alongside
Ringland Dental Surgery.

- Being lead nursing practitioner within the practice
- Making professional autonomous decisions for which they are accountable
- Receiving new cases, diagnosing, history taking, physical examination, referral and treatment including prescribing independently, follow up and discharging.
- Directing screening services within the practice
- Making differential diagnoses using decision making and problem solving skills.
- Directing care planning for chronic disease patients with existing nursing staff and administrative staff
- Ordering necessary investigations both individually and with the existing clinical team, including the doctors.
- Playing a supportive role in helping people manage and live with their illness
- Having authority to admit and discharge cases from their caseload.
- Working with other health care professionals and the PCN
- Demonstrate leadership and consultancy, and functioning to support more junior and administrative staff.
