Job summary
To provide
pharmaceutical support to Albany House Surgery.
The job holder is to work along-side
our GPs, Advanced Nurse Practitioner, practice prescription clerk and the PCN
Pharmacist to ensure patients are receiving the correct medication and reviews
as required.
This post is for 1 day a week (8 Hours)
Main duties of the job
The
key job responsibilities are:
- To work alongside
the PCN pharmacist on the Clinical Excellence and Investment Framework (CEIF) and
the Quality and Outcomes Framework (QOF) contracts.
- Be first point of
call for any medication queries within the Practice.
- To oversee repeat prescribing
requests and to inform and advise the current prescription clerks on best
practice and procedures.
- Review and screen
discharge letters to ensure the patients are being prescribed the correct
medication.
- To conduct
medication reviews on patients.
- To attend PCN
pharmaceutical meetings and practice meetings as required.
- To carry out
medication switches in line with local/national guidelines.
- Manage medicines
shortages by suggesting suitable alternatives
- Interpret national
and local commissioning decisions as they relate to medicines
About us
Albany House is a friendly, supportive,
and high performing Practice set in a beautiful grade II listed building in the
heart of Worcester. We serve a mixed patient population of just over 7000.
We
are looking for a conscientious, open and down-to-earth Practice Pharmacist to work 8 hours per week alongside our dynamic team. We want you to be passionate
about good patient care.
The friendly and open environment
in which we work allows us all to develop as clinicians. We hold regular
clinical meetings for all clinical members of staff to share ideas, best
practice and support each other. A sense of humor is considered essential!
Our team consists of 3 partners, 2 salaried doctors with
a great nursing team (made up of 2 nurses, 1 training ANP and 3 HCAs) as well
as an excellent managerial team and support staff. In addition, our local PCN provide us with support from a PCN pharmacist and a Pharmacy technician.

Essential
- The Clinical Pharmacist is enrolled in, or has qualified from, an approved 18-month training pathway or equivalent that equips the Clinical Pharmacist to:
- Be able to practice and prescribe safely and effectively in a primary care setting (for example, the CPPE Clinical Pharmacist training pathways)
- GPhC registered pharmacist
Desirable
- Hold or be working towards an GPhC independent prescribing qualification
- Minor ailments certification
- Membership of the Royal Pharmaceutical Society
- Working towards faculty membership of the Royal Pharmaceutical Society
- Medicines management qualification
- MUR and repeat dispensing certification
Experience
Essential
- An appreciation of the nature of GPs and general practice.
- An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
- An appreciation of the new NHS landscape including the relationships between individual practices, PCNs and the commissioners.
Desirable
- Minimum of two years working as a pharmacist demonstrated within a practice portfolio experience in managing pharmacy services in primary care.
- In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
- Broad knowledge of general practice.
- Experience of using Primary Care IT systems (we use EMIS, DOCMAN 10 and Accurx).
