Job Responsibilities
Work as a part of multi-disciplinary team (MDT) within
Grafton Medical Partners for Grafton PCN
Assess and triage patients, including same day triage, and as
appropriate provide definitive treatment following directives, NICE (national) and
local clinical guidelines and local care pathways) or make necessary referrals
to other members of the Primary Care and Secondary Care teams.
Advise patients on general healthcare and promote self-management
including signposting patients to the organisations Social Prescribing Service
and/or other community or voluntary services
The post holder is expected to be able to:

Perform health checks and reviews in line with local and
national guidelines

Perform and interpret ECGs
-
Perform investigatory procedures as required and ideally
be able to undertake the collection of pathological specimens including
intravenous blood samples, swabs and other samples

Support the delivery of Care Plans

Provide urgent and same day GP home visits for the
Practice
-
Communicate proactively on all levels within both
clinical and non-clinical teams ensuring effective, person-centered service is
delivered

Attend and contribute to the weekly in-practice clinical
meetings

Recording clear
and contemporary consultation notes as well as ensuring accurate completion of
all necessary documentation associated with patients health care records.

Communicate
effectively with patients and, where appropriate, family members and/or their
carers complex and sensitive information regarding their physical health needs,
results, treatments, etc.

A Prescribing Qualification is welcomed, but not
mandatory

Minor illness experience is desired, but not essential

candidate who has held a PCN role
We are happy to
recruit a Paramedic with minimum Band 7, working towards FCP training in
Practice.

any hazards in your working area are reported immediately to the Operations
Manager.

Ensure patient and Practice information is kept confidential and secure

To participate in mandatory training
sessions to ensure administrative, clinical and computer skills are kept up to
date with current technology and practice.

byCQC to ensure that the Practice is safe,efficient
andprovides the best possible care for patients.
To offer suggestions to the Practice
Manager on how we can improve, progress anddevelop allprocesses
relating to CQC.
