Aintree Park Group Practice is a well established GP
Practice in the north of the the city, and currently looks after in excess of
16000 patients, with a team of well skilled healthcare professionals as well as an administrative support team and
a team of Care Navigators. Supporting
the clinical team is the Practice Manager, Patient Services Manager, IT /Admin
Manager as well as the team of Care Administrators and Care Navigators. The Practice aims to ensure all patients are cared for, treated with dignity and respect and to help promote healthy living and well being.
Ideally you will have experience of working within Primary Care and have an ability to liaise with patients, staff and colleagues at all levels.
Main duties of the job
The successful candidate will be responsible for ensuring the delivery of safe and effective nursing care to the whole practice population across both practice sites. You will be able to demonstrate safe and competent clinical decision-making with a focus on monitoring of long term conditions and chronic disease management, immunisation programmes, cervical cytology, supporting the learning disability register, health prevention, health promotion and screening activities. The main focus of the role will be long term condition management but assessment of acute minor illness from initial history taking, clinical assessment, diagnosis, treatment, prescribing and evaluation of care will also form a key responsibility of this role. The post holder will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures and contractual requirements. The post holder will work closely with the lead nurse to plan and implement local and national guidelines, contracts and protocols, assisting in the performance monitoring and development of the service as well as helping to ensure the clinical team maintaining the highest professional standards.

The Practice is made up of 2 Partners, supported by salaried GP's, Advance Nurse Practitioners, Practice Nurse and Health Care Assistants.The Practice also has its own Pharmacist, and works closely with other health professionals - physiotherapist, health visitors, midwife, health trainer, all of whom have clinics within the Practice. The Practice has 2 surgeries, one in Orrell Park, Liverpool and the other in Old Roan, Aintree. The post holder would be required to work and travel between sites.
